Character: Ilun (tengu rogue 1)
Adventure: Burnt Offerings (Anniversary Ed.)
Catalyst: Ven Vinder
Technically not a DEATH obviously, but the only PC present was rendered helpless by an attack so I'm counting it. The setup is what you'd expect. They're down in the basement. He hears Ven coming. He hides, behind some crates in the corner with a fantastic stealth check... but not quite high enough. I decide to throw him a bone considering, have Shayliss try and distract Ven, give him a chance to sneak by along the wall when his back is turned... and roll a second 20 for Ven's perception. So he tries to talk his way out... but no luck there. So he bolts for the stairs. Ven catches him with an amazing grapple. He tries to escape... no luck. Ven gives him a single punch to the head, he's out cold with a point of lethal damage rolling over. Comes to in the middle of the road sometime later with his clothes in an unceremonious heap next to him.
Naturally, the first thing he did was arrange another date with Shayliss. Also, the PCs are now just kind of in this state of fearful awe with regards to Ven, coming up with crazy stories about how he must be some sort of legendary hero maintaining a low profile in retirement, or that he honed his fists on the jaws of a thousand suitors who would sneak off with his daughters. That's going to pay off later.

Story: The Yeth Hound's bay sent the fighter and the bard running, so it was just the Rogue, the Sorcerer and the Wizard. The hound took down the rogue to -3 hp (he did survive, though) while the wizard stepped forward to throw evocations at Nualia, but she made her reflex saves and hit him with the bastard sword for a ton of damage. Not sure why he only stepped back five feet, but the next hit took off his head.

Story: Xanesha absconded after grabbing the corpse of the beloved tagalong NPC she killed, and Damien followed. He made the checks to cast magic missile and kill Xanesha, but he had no contingency plan for the 16d6 fall damage. The party fighter got him a ring of featherfall promptly after raising him.
The first and so far only PC death in this game, and it wasn't even by my hand. Sheesh.

After a tough fight in the Lust wing involving a charmed shoanti fighter, horny summoned owlbears running around in a lustful frenzy and a traumatised azlanti warrior (alas, poor Mr.Mutt, innocent victim! Even years of captivity from a succubus didn't prepare him for that), the heroes were resting in the main room when an enraged Arkhryst found a way to enter the Runeforge to reclaim his treasure.
The fight that ensued was of epic proportions, and ended with two dead varisians whose fault was an adamant trust in their invisibility spells and the aforementioned charmed fighter breaking the bonds his own comrades had shackled him with to save the day. This was the tale of legendary Arkhryst's demise.

Cause of Death: In the encounter with the lamia, an injured Brutus is trying to flee down the treacherous stair in the Shadowclock to escape her wrath. The lamia leaps down from the stairs and uses featherfall to land beside Brutus. She then plunges her spear into his chest. He collapses and with no companions nearby, bleeds out dies from his wounds a couple of seconds later.

Story:
The party came up with an ingenious idea: They hired a new party member Annyo, a huntress with pet roc. Hal (the resident arcanist) prepped the party with protections from evil and a few other spells. They then had Annyo's roc fly them to the top Shadow Clock as night fell. Barely touching the belly of the Irespan they leapt from the bird, leaving it and Annyo to fly circles around the ruin incase they needed help. As they fell Hal called upon his magics and cast feather fall on the group. Needlesss to say Xanesha never saw it coming.
The party knowing about Xanesha, and her abilites from a recently freed Ironbriar, made sure that her casting spells was near impossible readying actions to interrupt her casting. This caused the lamia and go on the offensive. First she nearly took out the party goblin and rogue Smiley. Julie had him fall back and healed some of his wounds. This made her a target for the lamia from them on. Even with the clerics prayer spell and Hal's protection from evil in effect the spear made quick work of her. Luckily with her last breath she summoned up help for the party in the form of three lantern archons.
A crash of lighting, a falling statue, and two powerful "angelic" blasts later, Xanesha was no more.

After the passing of the cleric, Hal took desperate measure to kill the lamia. He summoned a bolt of lighting and targeted the rickety supports hold the angel statue aloft on the clock tower. It came crashing down on Xanesha and Merisel, while also causing a large portion of the floor to cave-in. Xanesha was wounded, but managed to leap to solid groung. Merisel wasn't so lucky. She plummeted to her death followed by statue, and clock bells, all the while cursing Hal's name.

Story:
In the midst of a howling snowstorm, the party anchored themselves to Halek, their stalwart barbarian, as he used his +33 Climb roll to scale the 2000' cliff to reach Karivek Vekker's resting place. As they reached the top, they heard massive cracks in the fog-shrouded ice in front of them. Not knowing how to react, they prepared themselves as best they could, but the ice worm got the drop on them and breathed on them. Shiro the bard Jester's Jaunted them over to the worm, and Halek and Rae'Sheleth the paladin made short work of it. Unfortunately, it exploded, and Hi failed his Reflex save.
He was still up and in fighting form when Karivek's ghost arrived to attack. Unable to reach Halek to start devouring him, and deeming Hi too small of a morsel to bite, he instead chose to use his corrupting touch.
A grand total of over 50d6 of damage finally did in the stalwart gnome.

Two hits from a horsechopper later, the party started to wonder what that sickening wet shanking sound was, and urgently began to tear down the wall. They spent a few desperate rounds ripping chunks out of the masonry, and firing arrows randomly into the wall in the vague direction of where they thought the goblin might be. All to no avail. The cornered goblin had found nothing more amusing to do back there than to repeatedly coup de grace the fallen PC.

However, the sinspawn manage to force Rock into the runewell and the party ran like hell. While Rock took on the last sinspawn the party shut and locked the door behind them. They opened it to find that Rock in shredded pieces spread across the room. Assuming he was stronger than the sinspawn, they forgot to consider the damage he took from the well. May he rest in shreds.

Our Rogue made the mistake of entering melee after being healed up only slightly from near death. He was the only one in melee range for the ogre, so the ogre takes a full round action. 24 damage on the first swing, crits for 41 on the second swing. Dead rogue. My first PC death.

I can't wait for my players' party to get really started with this AP (they are currently just about to learn about the glassworks problem), as so many of them them have dump-statted their characters' Wisdom to 8 ... :)
Keep them alive by hook and crook until they get to certain WIS/Will focused villains ... ;)

Had my first PC casualty this past weekend, though he wanted to introduce a new character as he was quite bored with his Slumber hex witch (as I expect happens to most witches who take this hex and don't plan ahead to make themselves viable in other ways), so it was kind of a gimme... no unexpected deaths yet, sadly. ::GM sigh::
They managed to fight their way through, meeting little resistance. Little did they know, Ripnugget was on to their plan and some of the goblin contingency had already relocated from the guard towers and sleeping quarters to the throne room. Side note: I threw in some extra goblins in this scenario because my PCs are pretty good at what they're doing and at this point, goblins are kind of a cakewalk for them.
So there they are, ready to parlay with Ripnugget, the witch standing alone at the front of the room with the rest of the party backed up against the wall, when 9 goblins drop down from the ceiling, completely surrounding the witch. Surprise round, each goblin gets a whack at the witch and after the fourth goblin, the rest of them start dismembering the witch's body. One goblin ended up wearing the witch's entrails as jewelry before falling to the replacement barbarian (who had tracked the party down by following the trail of dead goblins up to this point).
Of course, now Ripnugget's been taken care of and the big battle is over... or so my PCs think. They haven't even delved into the dungeon layers of Thistletop, where I suspect several more PC deaths before the end of the chapter. I just hope my players aren't too upset about the whole ordeal... though, I did warn them to stock up on potions and magic items... they seemed more interested in hoarding their gold for later.

1) No Mage Armor or Shield, for 26 AC after Haste.
2) She opted to do non-lethal damage (-4 penalty on all attacks) in hopes of capturing some useful new minions to charm.
The party still had a very rough time of it. About halfway through the fight, Azaelyn wanted to stick a Wand of Fireball into Xanesha's mouth and shout the command word. I made the PC roll an attack -- came up 28 -- and then spell resistance -- came up 16, but I gave a +2 awesomeness bonus. And since the fireball was contained inside Xanesha, I ruled that she took +50% damage. It was the single biggest hit of the entire fight.
Naturally Xanesha was thoroughly pissed off, so she decided to quit dealing non-lethal damage and just kill Azaelyn outright. Which she did. In one full-attack.

In desperation, Jubrayl picked up Azaelyn's Wand of Fireball and started using it. (He has Fire domain, so Fireball is on his spell list -- no UMD check needed.) Orik took 2 without dying.
The fight concluded when Jubrayl announced he was going to pray to Sarenrae and leap off the tower in the hopes of surviving the fall and rescuing or avenging his friends. Sarenrae answered his prayer by having him fall into a portal and appear behind Xanesha, fully healed, with a +5 sacred bonus on basically everything for 3 rounds. On the third round, he tried the Wand of Fireballs one last time and finally made the SR check. That fireball dropped Xanesha below 20 hit points, but killed Orik outright. Xanesha fled.
So, that was my Xanesha fight using the 3.5 variant. I nerfed her a good bit, and it STILL required direct divine intervention to avoid a TPK. Jeez. I'm just glad the cleric actually prayed to his goddess before jumping. If he hadn't, I'm pretty sure the drop would have killed him, and I'd have let him die.

Little confused about this part. If the AoO on the coup de grace destroyed the ghoul, the alchemist should have been fine. AoOs interrupt the action that provokes them.

Having chosen a summoners build that left him a partial outsider he was trapped in the circle. The Devil was willing to deal for both their release, but Arlen asked the party to not do it. He would rather die trapped here than let the Pit Fiend escape.

Story:
Upon entering the Chapel, one of the two Yeth dogs bay, causing 2 of the 6 party member to flee in terror. The wizard finds his way randomly to the upper surface and cowers with the rescued horse (who looks at him in disgust). The Monk randomnly runs into the cave lurker who grapples him and proceeds to consume him, while the remainder of the party struggle against the hounds.
The party attempt to close the doors on the Yeth Hounds, but the Quasit Familiar/witch that had escaped from them under the Catacoms of Wrath has taken refuge here, and moves forward and opens the door allowing the hounds to persue the party into the hallway.
It takes about 6 rounds, but the party eventually dispatch the hounds almost killing two of the remaining party members, who go unconscious.
With party members down, the two remaining members take time to heal the downed party members and then go and seek the Monk, whose dead body is drug out by the cave lurker out to the cliff edge (The beast snarling 'Stay away from my meal!'
The party use the cave entrance to lower the horse to the ocean, drop themselves via rope to the horse and flee the scene, to return later.
The goblin druid is ecstatic to have achieved chiefdom of the Thistletop tribe and takes control of the fort and rebuilds the bridge.

They cannot speak, but possess an eerie form of telepathy that works only upon creatures they are in physical contact with—a feature they often use to “chat” with their food as they eat.

This is a death of sorts.
At the end of the 2nd part in this AP, the party had thanks to some choice comments from a party halfling rogue named Shiflett almost compromised their mission in exposing the Skinsaw Cult.
The exact words escape me, but it basically implicated them in the death of Justice Ironbriar and the murder of people at a sawmill.
It wasnt looking pretty as the party escaped and managed to convince the powers that be, that it wasnt them and they would sort the matter out by defeating the BBEG in Magnimar.
We managed to pull it off, and the player playing Shiflett decided he wanted to try something else so made a new Halfling Ninja.
As we are taking it in turns running each part of the AP, it was now my turn to run Hook Mountain.
Basically the City wanted someone to pin the blame on and Shiflett was an easy mark as he had been known to do some downright nasty things. The cultists and ironbriar was dead so it couldnt be them. During the adventure, word trickled out via communication with Magnimar that Shiflett was found guilty and to be hanged. I havnt gone through with it, but it seemed a fitting end for a character who dropped the party in it with wayward mouth on several occasions.

A little while later, though, the band discovered the hounds of Lamashtu. One of the hounds won initiative and bayed. Almost unbelievably, the fighter, ninja, wizard, and wizard's cat familiar (Andrew) all failed their saving throws, leaving just the cleric in control of herself. She only managed to catch the fighter with Calm Emotions, while the other three fled blindly through the shrine. None of them fled from the shrine itself, and the Scribbler seized the opportunity to send the two babau demons that he had summoned to attack the ninja, while he and a hound followed Jake and his familiar. They were cowering at the end of a hallway with no other apparent exits. It took virtually no time for the hound and the Scribbler to shred Jake and Andrew.
Happily, Jake has since been raised and has summoned a new cat familiar, Andrew, Jr.

Catalyst: Not being as sneaky as he thought he was
Story: So, after mucking about in the Foxglove Mansion for a while and experiencing a couple of the haunts, Brunkel fails his save on the haunt in the den that has him suddenly believing he is a mother desperately trying to save his/her child (randomly determined to be the dwarven cleric Tungdil), and pulls Tungdil along outside to the ruined outbuilding. Tungdil goes along because he is curious where the haunt is taking Brunkel and why. The rest of the party trails along. Cue carrionstorms, which Brunkel takes the brunt of, but which everyone survives easily enough. Then the party decides the ghost must have been trying to tell them something and decides to go down the well, sending Brunkel, who was only partially healed from the carrionstorms, down first. After he and Spider, the monk, get down, and while the others are still coming down the rope, one by one, Brunkel decides to take advantage of his darkvision and explore, leaving Spider alone in the dark (Brunkel had the light source in his pack). He gets to the ghoulish dire bat room, spots the critter hanging from the ceiling, but doesn't know what it is. He decides that he will try and sneak past it to explore a bit more. He rolls a 3 on his Stealth. Direbats have pretty good Perception scores. He is detected and it attacks and puts him down in two rounds, long before Spider or anyone else can come to his rescue. Moral of this story - don't try to sneak past a bat in the dark, and don't leave the only person who can help you quickly without a light source.

After clearing the ogres from the first floor, the party quietly headed up stairs. Entering the large hallway, Hal the arcanist used his magic to hold the southern door tight in the hopes of preventing ogre back up. They found "Papi" praying at the desecrated Black Arrow shrine, and ook the opportunity to attack.
At first it seemed like they were going to win this fight easy; Landing plenty of hard hits against the ogre patriarch. Then Papi began swinging his ogre hook. Hit first attack (a crit) smashed in the head of Abigail. Papi remarked that after this was over he woulf use what was left of her head as a codpiece

So, the party is chasing the female wizard who is retreating to Nualia. Barbarian notices the polished section of the floor, Jumps across informing the party. Bard Jumps across. Cleric in Banded mail Jumps across. Summoner Jumps across. Witch rolls a 1, fails Reflex save. She didn't heal after facing the bugbear and only had 10 HP when the glaives went to work.
The party had been feeling nigh unstoppable at this point, even with me increasing the difficulty of the encounters to compensate for the 1d12+13 Damage Greataxe barbarian. (24str Raging, power attack).
This made them all afraid.
"You mean she... she's dead? like.. dead dead?" Game got serious REAL fast.

I finally killed a couple players; they managed to make it all the way to book 4 before any deaths, although their close calls can not easily be counted.

For some bizarre reason, my group of 4 players decided to put one character at each entrance to town for the giant attack, so they were all split up and facing down encounters alone or with a few token NPC helpers. Frankly it is a wonder they didn't get TPKed by doing that. They eventually got themselves united, but it was too late for Amelia, who was finished off by Teraktinus a couple rounds before he went down himself. This poor defensive tactic (seriously they could have just had the witch dimension door them around as needed) ended up costing them the brewery and Scarnetti manor and a number of kidnapped citizens.

Wasn't a good day for poor Usjach, who barely survived a 101 point crit from the stone giant general's heavy pick thanks to a good false life roll from his Sihedron medallion, only to later fail a hold person save on a 1 (the only way he could fail, being a paladin with an extra +4 on his will save due to and get himself coup-de-graced by one of the lamia sisters.

Healing cleric had CLW, CMW, and at least 1 channel remaining at the time, but was engaged on the other side of the room.
BTW, this is my first killed PC since I re-took up gaming. I don't feel particularly good about it, but it was a dice fluke and a miscommunication about healing needs, not an Evil DM that did poor Thorn in this time (ok, not just an Evil DM...)
