Name of PC: Light Silverbow
Class/Level: Human Divine Hunter Paladin (8)
Adventure: The Hook Mountain Massacre
Catalyst: Jaagrath Kreeg

Spoiler:
The party had managed to bypass all the ogres on the ground floor of Fort Rannick the party made it to the first floor. And charged into the Chapel. They managed a surprise round on Jaagrath and then the fight started proper. Light 5'ed into the corner of the room to provide supporting fire and unleashed a volley of arrows. Well, it would of been a volley except the corner was within reach range of Jaagrath. Can you say Attack of Opportunity? I can.
Light: "But I'm not standing right next to the Ogre"
Half-Orc Ranger: "Is ogre, has reach."
Light: "Oh cr@P!"

In addition to saying AOO, I can also say Threat and Confirmed Crit.
With a human bane x3 weapon. Over 100 damage. Light went down.

Name of PC: Fang
Class/Level: Wolf (8)
Adventure: The Hook Mountain Massacre
Catalyst: Ogre Fighter

Spoiler:
The above battle that saw Light sliced open by a hook kinda spiraled rapidly out of control, with Ogres coming at the party from all directions. The party wizard placed a Spiked Pit at one of the doors, catching an Ogre who would spend most of the rest of the battle trying to get out. Towards the end of the battle, an Ogre Fighter bullrushed Fang, the Half-Orc Rangers pet wolf into the pit. The fall was enough (with the previous damage done to the wolf) to drop Fang to negative hit-points. The Ogre at the bottom of the pit now had something to kill and eat while waiting for the pit to end.

And a near kill:
Name of PC: Thumper
Class/Level: Human Aegis (8)
Adventure: The Hook Mountain Massacre
Catalyst: Several Ogre Fighters

Spoiler:
Still in the same fight, the Aegis, who had been happily blocking the stairs finally ran out of power points and was taken to negative hitpoints. He didn't die, due to a quick stabilisation. But only the previous round, he had be happy to stay in the fight, rather than running away from the fight. But the party managed to get him out when they ran away, uh, retreated to regroup. I'm actually kinda glad he did survive, since I had just killed the players previous character in the basement of the Graul Homestead.

Entertainingly, the only people to fail the Will save against Dorella's confusion spell where the people with the best Will save.

Just burn it to the ground. :)
So often, that would have been my solution to so many problems, but apparently Varisia is a lot like Oregon; a wet desolate wilderness where nothing can burn, even when it should...

One of the things I've been emphasising to my players since they arrived at Turtleback Ferry is rain. It started a couple of weeks before they arrived, it will carry on until sometime after Skull's Crossing.

As a happy coincidence, that made the farm far to damp to burn the ground.

Name of PC: Jaina
Class/Level: Human Monk 7
Adventure: Hook Mountain Massacre
Catalyst: Poisoned then hooked
Story: The party had learnt to hate the Graul Farm, they had managed to trigger all but two of the traps dotted about the farmhouse (and since they where an oversized party (7 to 8 players on average), there was a few extra traps. But that was last session. This session, there was only four players. And there was only one trap left, the poisoned trapped in the chest in the attic. Jaina was volunteered. And failed. Lots of Str and Dex damage occurred. Not fatal, but annoying.
Fatal happened in the basement, where the Dex-reduced Monk whose Mage Armour spell had expired, went from almost untouchable to really easy to hit. Which a raging Hucker Graul demonstrated with his +1 Ogre Hook several times, including the crit that tore poor Jaina apart. Didn't even have a chance to bleed out.

This is the second time I've killed this players character. I might need to spread the kills around a bit.

Name of PC: Yari
Class/Level: Lvl 7 Barbarian (grappling specialist)
Adventure: The Skinsaw Murder: Shadows of Time
Catalyst: The Scarecrow
Story: Honestly, I didn't think the Scarecrow would manage this, but against an over-sized party (there are 9 players in my group) but, attacking from hidden, the Scarecrow managed serious amounts of damage to a sorcerer and a cleric.

After the cleric and the sorcerer managed to fly out of the Scarecrows reach, it turned its attention to the next target, a monk (the replacement PC for the player that lost a wizard earlier in the campaign) and took the monk down to negative hit points (at which point, the PC expected a repeat of his wizard and started rolling a replacement character). But this wasn't to be the monks time.

Yari the barbarian, raging at the near death of his companion (or not - he'd probably be the first one to loot the body) charged and grappled the Scarecrow, and the cleric and paladin brought the monk back from the brink of death. But the Scarecrow managed to escape the grapple and with one might swing of it's scythe (nat 20 and confirmed crit) eviscerated the barbarian. The rest of the party panicked at the death of thier comrade escaped up the tower.

Four weeks to the next session and the big showdown.

Some six months ago:
Name of PC: Never got as far as a name
Class/Level: Elf Wizard 3
Adventure: Burnt Offerings
Catalyst: Bruthazmus, the Elf hating Bugbear, with the heavy flail.
Story: The party are exploring down a corridor, and the thief fails a stealth check big time, allowing Bruthazmus to get the drop on them. Bruthazmus does major damage to the thief in the surprise round. The wizard (top of the initiative order)dashes forward to give resistance to the thief, who promptly and sensibly tumbles as far away from Bruthazmus as possible. Bruthazmus, with no other target, steps up and crits on the wizard for some 30 points of damage, turning the wizard into an interesting splatter pattern on the wall.

The last session:
Name of PC: Albretcht
Class/Level: Fighter 5
Adventure: Skinsaw Murders
Catalyst: A well.
Story: The party are about to leave the Foxglove mansion, where they have had a really rough couple of days. For some reason, they are really worried about the Carrionstorms. Albretcht (who has taken from serious WIS damage) decides to lead a heroic charge and charges out. The rest of the party decide to stay exactly where they are. Albretcht panics and (after I double and triple check with him) decides the quickest escape from the Carrionstorms is to leap down the well. Lots of D6s are rolled. Albretcht was technically correct; the Carrionstorms no longer both him, due to the terminal impact injuries. Cue the rest of the party launching a heroic body looting down a 100' deep well while under attack from Carrionstorms.
