Lucien suggests they leave the dark altar alone for a while, and leads the group back underground, towards where the statue of the angry woman with a magic pole-arm stood. Before he goes into that room, he tiptoes down the rocky cavern to the north, to see what else happens in that direction, just to satisfy his curiosity.
He is +9 Stealth if required

Mira leads the group back down below, pausing to let Lucien sneak about to see if he spots any signs of trouble.
"Come right back if you see anything dangerous."

Heading down the long hall the stairs descend into the depths, gradually, but certainly downward, until they reach a door of intricately carved stone, with runes lining the edges, similar to the first door you entered through and the double doors that Lucien discovered in the altar room...

"Wait. Now that we know what's down here, perhaps we should finish up the upstairs before venturing forward. Might be important clues or even worse something lurking there that would prevent our leaving".

"Snag think we finish here. Not keep going back and forth." Snag says.

Lucien is ready to open the door with the arcane markings when Mangrove says hold. Then Snag suggests he go ahead, and so he does, opening that door at the bottom of lots of stairs, as quietly and carefully as he can.

Lucien reacts quickly to the winged head, braving its initial effect, he fires twice at it with his shortbow. But hitting the target seems a bigger challenge, and his shots both go wide.

Is this one of those RARE instances in PF2e where there is a surprise / prepared action, or did this happen on the creature's initiative.

One of those rare instances... ^_^ I expect that you all will make short work of this critter.
Lucien, quickly shaking himself free, fires two quick shots with his bow, but the flapping wings of the bizarre creature manage to carry it out of the way!
Before it acts, I will wait for Mira to roll her save...

After the flying head bites down on the halfling's neck, tearing a small chunk of flesh free, the grizzly creature attempts to fly back to the dark ceiling!
Anyone with Reactive Strike may take it now. The creature is now 15 feet up, and considered concealed unless you have low-light vision or darkvision.

Mira grits her teeth as her warhammer slams into the stone floor with a loud crack, the flying monstrosity already out of reach. The shriek, the flapping wings, Lucien's cry of pain-it all crashes into her senses at once.
Her muscles tense to swing again, but she knows the weight of the warhammer won’t help her now.
Too slow, Too heavy.
Just for a moment, the thought of Salah’s words and the graceful curve of the scimitar flickers through her mind. Faster, lighter, better suited for something like that
She pushes the idea aside, focus narrowing in on the immediate threat.
Without hesitation, Mira drops the hammer, her hand already reaching for her crossbow slung across her back. She brings it up, cocks the mechanism and says "Let’s see how you like this!"

Mangrove spits a seed pod at the hiding creature! As it explodes upon the stone ceiling, debris rains down upon you! It is not entirely clear whether it found its mark, but Mangroves song fills you all with hope and boldness!
Snag steps up and sends out an arc of lightning into the darkness at the black silhouette! Ref Save: 1d20 ⇒ 7 Fail! -7 hp! The flying creature is lit up by the arc, leaving it smoking, but still moving!
It flaps down again, descending on the little goblin this time! Stride, Strike, Stride

Mira quickly reloads her crossbow, searching for the creature and fires another bolt, but this time she does not take her time to aim and misses.
"Hang in there!" she calls out to her team as she quickly loads another bolt.

Anthony, are you striking at range? It flies down, bites, then flies back up to the ceiling. As Mira is using a crossbow now, no one is able to make a Reactive Strike. You can Ready an Action for 2 actions to swing when it flies down again, if you wish to attack it at melee...
