The first time it occurs in the scenario, it's described as a series of small encounters, with each one doing 1d8+2 at this tier. The second time, with Swareti and the slaves, it says "PCs who participate in running a diversion take 4d8+8 damage from the duergar they distract in Subtier 5–6, or 4d8+20 damage in Subtier 8–9."
So it could be interpreted the same as in the first instance, but it's not explicitly written that way. The damage scales right in each tier for the distraction effort to count as 4 separate encounters, but a GM who didn't think about that or agree that that's what is intended could easily kill someone.

As soon as you get back to the lodge, a portal in time opens and sucks Sard in. He pops back into view a moment later looking weathered and quite a bit more experienced!
This scenario leveled Sard to 6th, then the three GM credits for F2F games I ran in the interim kick in, leveling him again.

Ostendiel had a similar experience... only he leveled with this one, and then all of a sudden went to the Plane of Air to bear keeps on clouds, or something. (A game I GMed while Ostendiel was in the middle of this game. Or maybe before? He wasn't the right level yet if so.)
