House rule? I thought that PC checks were just the PC and only NPC fumbles could be activated by any PC with action dice. Or is it a class ability?

If Darius or Darius & Marco move 5' to their left, Crow will be able to flank with Darius the following round. On the other hand, if Marco is going to continue swinging away Crow will just step in where he can. No worries.
If you remember looking at an old style (non-digital) clock, clockwise refers to the direction the long and short hands both sweep in.

Logan: in regards to your out of character posts. According to the notes on Crow's sheet, Barron was NOT an Animal Partner. He was a normal crow.

Since we were holding at the door - should our actions have triggered as soon as we heard Sebastian screaming and thus had a chance of saving the innocents given the action economy in FantasyCraft?

Let's see, Marco or Crow could open the door, Darius could hand off the doctor's bag, Mead could go to assist the innocents, and the remaining Crow or Marco could move to block Sebastian. If we get our actions that way, it could work.

But it take an action to take out a vial and another to get there, so no chance to save even one, much less two. Mead is a character that cares about innocents and is not happy we were not able to act to try and save them.

Well, if they failed their save, they're dead anyways, I assumed that we were going to use the doctor's bag to make sure they don't die and then go about protecting them. Or if we could break in during his original speech about fools and assassins and plot and ploys. In any case, it was more about if we actually counted as readied (or holding or whatever) and when we would get to break in if so. Somehow I assume that we're past that chance anyways so...
