There is already a mysterious 5th horseman called the Oinodaemon. Not much is known and I am REALLY hopeful that with Mythic gameplay we'll see an AP exploring the concept.

Despair.
Granted in the current state of the Oinodaemon madness seems appropriate.
Prophecy might also make some sense as the Oinodaemon has uttered prophecies...ones that I think even scared the other horsemen. So Despair again.
Also, I guess prophecy about the end of times etc. is kind of fitting.
I'm also wondering if rebirth makes a slight bit of sense. As the horsemen destroy which gives way for something new. However, that makes no sense for a Daemon. Unless that is why they turned on him in the first place. I doubt it.
I'm thinking more Terror, Loss, Forgotten, Forsaken, Finality, the end...Despair.
I guess if I say every word in the English language maybe I'll get the right one.

If there was a tangent point for Obsession/Despair/Hunger/Agony you'd have the Oinodaemon, at least in my headcanon. :D

The NPC |

Destruction can't be the 5th because each horseman represents some aspect of destruction. Unless of course this theoretical 5th stands above the others. It has to actively destroy...
Someone suggested Chemos, destruction via substance abuse and addiction.
Rick Flair had to give the horse over to Charlotte.

We have War, Death, Conquest/Pestilence, and Famine.
What would you posit for a 5th Horseman?
{boggles} Really?! You still haven't figured it out? Wow, I've never seen any CritFail their Knowledge, Perception, and Sense Motive checks that badly all at the same time.
C'mon, you seriously don't know Who?

We are talking about the Horsemen, not the Apocalypse Himself.

Necromancer |

The fifth Horseman, Socialism, actually comes first and doesn't present itself with a horse. It hides the horse immediately after spawning and carries two coconut halves. It bangs these together and truthfully says that it is a Horseman. Then it proceedes to tell everyone else that they have horses of their own. Many believe this and collect improvised percussion that mimics the sound of hooves. Following this nonsense, Socialism tells the Patsies that they are now mounted cavalry units. Once this insane mob has been blooded sufficiently, Socialism mounts its horse and insists that nothing has changed. Afterwards begins the great "false equine cleansing" where the mob murders any surviving horse for commiting the heresy of existence, thus paving the way for the other Horsemen.

~LAUGHTER~ I would vote for him. He can't be any worse than anyone else.

Thinking of things that feel thematic to appear in an 'of the Apocalypse,' I'd go with Disaster/Cataclysm (earthquakes, tidal waves, floods, volcanoes, hurricanes, rain of fire, rain of toads, rain of toads that are on fire, etc.).
Despair and Madness also sound appropriate.
Misfortune / Doom / Curses could be different.

It's fascinating that the horsemen are never given roles in the book of revelations. They have only a colour, and a "tool of office". They have certainly been interpreted differently at different times.
I liked the version that painted the top of the RCC as three of the horsemen (courtesy of Jack Chick - the RCC is the only thing he hates worse than D&D, probably): The pope (white) sending arrows of plague across the world from his bent crucifix and wearing the papal crown, the Jesuits (black) controlling every little part of the markets and the companies that provide stuff to buy, and communism (red) because, well, communism is evil, duh.
