Cause of Death: Radiation!Furkas Xoud prepared the deadly technology guide spell irradiate twice, and in his third and final confrontation with the party, he lasted long enough to cast both and deal a total of 11 Con drain to several PCs. That was enough to kill poor Erwick the halfling bard even though he’d avoided taking any hp damage in the fight. Alfonso the varisian magus would have survived if he hadn’t been injured by fire and lightning in the preceding rounds.
Irradiate is an absurdly deadly 3rd level spell for high level casters. Once you hit 17th and get the full effect it's just gross, then you can add on empower or maximize metamagic to make it an AOE SOD.

After a short but well-fought fight, one of the other party members landed a crit while grappled by Hellion's pincers, dramatically ending the battle...and his friends lives.

The bed disassembles him with ease. Forceps tug his arms aside while a beam of red light flays him open. The "red food" goes everywhere, I'll have to remember to share that with our acquaintance. His heart stops, I hear that, and I still hear Khonnir screaming like some caged animal.
We get back and the rat makes us breakfast, I go to tell him that it's dinner time, but think of better of it. He does what he wants. Instead I leave the Foundry making my way to Jorum's place, they've got Khonnir there. He still looks bad, I don't have words to describe it, but it's bad. So I sit by his side while Val's sobs make for some pathetic white noise, at least it lets me think.
I'm vaguely aware of the time passing, more inclined to reflect upon the past. Kilvin wanted to return, but that made no sense. We were close and there was blood and I wasn't sure what that meant. I needed to press and no one stopped me, no one even tried. Even Kilvin who wanted to return doesn't try. So we press on.
There are two robots they're each the size of a horse. We back into a bathroom and find our pace, but they're tired. KEP/DAIV/whoever he is now isn't, but the rest are. I've heard about elven frailty and now I've seen it, Arato collapses in a heap and the "medical" robot drags him back to the bed. The bed, that damn bed, disassembles him with ease.
Just so it makes sense there was some plant creature we befriended that wanted to eat red food (blood). The rat is a Ratfolk Gunslinger in the party who fancies himself a chef. KEP/DAIV/whoever is an android in our party whose taken on a few names so far. And Kilvin is also a party member. This is written from my character's perspective, who is also an Android which is why things are described kind of distant.

The Gory Details:
The party is exploring Xoud's bedroom when Kalium decides to try to tear down a tapestry at the same time another character finds Xoud's spellbooks. Infuriated, Xoud manifests and Kalium borrows luck from the future to reroll his initiative. Kalium winds up going first and wraps Xoud in a wall of flames, even though Xoud had previously fireballed himself and the party to reveal his fire resistance, which prevented the party's melee characters from moving in to assist him. Xoud cast phantasmal killer and proceeded to fail both saves due to the reroll. The party rogue managed to drag him back to the cleric and hunter, who used a trauma pack plus to restore his life, so Kalium was plucked from the river of souls before he got to find out which deity had saved his life by granting him the curse and mysteries (back story stuff).

Indeed... Ranger hating humans/elves, plus gravity bow, plus a human bane arrow = Trouble.
So Slaid, the Steel Hawk commander, convinced the party to take back her old headquarters.
Their plan was simple: Walk up to any guards under the pretense of diplomacy, and give them all the beat down. She gave them a layout of the building, where the guards would be posted, where the additional guards would come from (the barracks), and where Birdfood would be (upstairs in the throne room, overlooking the entrance).
The PC's walk up all confident as the two orc guards tell them to surrender their weapons and initiative begins.
The party's elf archer fires two arrows at Birdfood, hitting once, for 12 damage. The party's gunslinger shoots and kills one of the two guards. Birdfood then fires once at the "human" gunslinger (who spends a grit point to fall prone and dive out of the way of arrow), then a second arrow at the elf archer, which hits for 28 points of damage, taking him to 0 hit points. His third arrow misses. The android fighter moves up and kills the second orc guard. The half-orc cleric channels and heals the archer, while the investigator/wizard readies a concussion grenade.
The elf archer cries in pain, and dives behind physical cover (taking no fire, nor receiving any). The gunslinger readies to shoot when/if more orcs approach. Birdfood takes a shot at the human gunslinger, who spends another grit point to go prone to avoid getting hit. Running out of human bane arrows, he continues his onslaught against the only remaining human opponent: the investigator/wizard. Two shots later, the wizard is dead (34 hit points in two shots... dice were really hot last night). The 6 orcs open the door to the barracks and engage the party.
The party easily advanced through the orcs, around towards the rear-throne room, when Birdfood's hawks engage the party. Sensing his death should he stay put, he climbed out the window (the party was already making their way around and clear of the center courtyard), and down to claim the dead wizard's body as a prize to Hellion.
As he's dragging the body away, the party approaches the throne room, only to find it empty (and all of a sudden they get that sickening feeling that someone should have stayed with the wizard). They look out the window, and notice that the Congar is not there...
They follow the blood trail to an abandoned building where another faction (the Knights [a faction of my creation]) killed Birdfood and claimed the wizard's body as theirs. The PC's were able to successfully negotiate for the return of the wizard. Afterwards they had Dinvaya Lanalei successfully use the Resurrection scroll upon Congar, who returned to the land of the living...

The Gory Details: The party survived getting force walled and cloud killed in Furkas' bedroom, stole his spellbooks, and found the stairway leading down. They decided to go down instead of clearing the top of the tower, so they fought the Tought Harvester, Furkas, and the Robotic Apprentices (they joined the battle a handful of rounds into the combat. Despite the amount of forces arrayed against them, the PC's were wearing the enemies down. Furkas, running low on spells, decided to go for broke with a Phantasmal Killer on the party rogue, who promptly fell over dead. Once the fight was over, they were able to get their buddy Longdreamer to give them a quick lift to Hajoth Hakados to get their comrade raised.

The party took his corpse back to Torch and immolated it in the torch.

The party was fighting the followers of Hellion, a mixture of orcs and acolytes and those pesky dark folk. One round after the action began, a roar of a chainsaw bellowed through the chamber. Many of Hellion's followers fell. The next round, Kulgara ran in with a bloodthirsty grin on her face as the minions chanted, "Kulgara carves them up!" Kulgara ran up to the party oracle and nearly cut her in half. As the oracle ran away on her turn, she provoked an attack of opportunity from one of the lesser orcs, who finished what Kulgara started with a lucky hit and, like, 6 DMG. Next round the orc used the oracle's severed head as an improvised weapon to bludgeon the party hunter, who killed said orc promptly. The party witch (see below) nailed her slumber hex, and Kulgara ate a coup de grace from the party magus.

Catalyst: Hellion. Also the witch.
Explosions everywhere. The witch first used a zero grenade on Hellion to great effect. Next, she had the bright idea to use Kulgara's plasma grenade on Hellion. Hellion soaked the AoOs as he used his second initiative pass to grab the grenade and lob it at the party. The witch died right away, the magus was unconscious and dying, and everyone else got hurt. The hunter and (resurrected after the above death) oracle finished the fight, with some noteworthy help from the hunter's mammoth companion, at which point Hellion talked some smack and went boom. The dying magus became a dead magus. The oracle was 1 HP away from death, and the hunter 1 HP from unconsciousness. Close to a TPK. Brutal, brutal fight.

Gory Details: The Lukwata used Awesome Blow on Volga, then crit Volga for 102 damage as he moved back into melee putting him at -40ish. Val (who is a Cleric of Brigh in my game since none of the PC's wanted to be the healer, and a recovering Numerian Fluid addict because because why not) came out of delay and made a sick diving touch to deliver the BoL, then held the now unconscious Volga's head out of the water. Morgrim then proceed to finish clawing the Lukwata to death with Termo's assistance. So, Volga got BoL'd, but I'm gonna count it. Lesson learned? Awesome Blow is rad!

Gory Details: The party were investigating the Aurora, and Aliss managed to set off the malfunctioning doorway taking a large amount of damage. As the cleric is also the one who hand's the disabling or traps, by the time the trap was dealt with, they'd forgotten to heal the magus, and the party continued on, running into the Androids.
By the end of the fight Seerath was alone and trapped in the corner, and all the party was moving forward to fight her... providing a perfect set of targets for a grenade. A failed reflex save and the damage from earlier left Aliss with one hp off death with her turn coming up next where she failed to stabilize.
Luckily for her the party have been competent till now so still had the scroll, and Aliss is back for round two.

Half a tank of gas and a fully loaded c**k, the party continued deeper into the depths of the machine.
The PC arrived at the base of the machine, near the central programing room, when the dark stalkers began to lay down zones of darkness. Clearly this meant that the PC’s were meant to follow the darkness (a.k.a. the carrot-and-the-stick), which lead them into the large main room.
Waiting there was the Ettin, and Kulgara, with a couple rounds of prep, and the PC’s in a five-foot hallway as the first PC (fighter-archer) enters into the light. Combat begins with the Archer winning initiative and running back into the five-foot hallway.
The raged Ettin winds up on Defender and hits a couple of times, softening him up for Kulgara. Kulgara finished Defender off in round 2 of combat. The party attempted to adjust their lineup, but it was too difficult being stuck in a five-foot hallway (between the control room and the main hall).
So half of the party is up front (fighter, fighter, archer) trying in vain to defeat Kulgara, while the other half outside the five-foot hallway (cleric, gunslinger, wizard) is screwing around with the dark stalkers daggers that had the darkness spells placed on them.
Argos II steps up into the hole to give the other PC’s time to formulate a plan. The archer lays a few arrows into Kulgara, and on round 4 Argos II takes a nasty crit from the chainsaw, dropping him into the negatives. The archer puts a couple more arrows into Kulgara, but doesn’t approach in melee. The cleric is out of range as the PC’s remove the rest of the darkness daggers. Kulgara, with no other melee targets, digs deep into Argos II’s intestinal tract, killing him.
The gunslinger cycles through three different timeworn weapons to finally take out Kulgara from range, and the Ettin is finished soon afterwards.
The PC’s retreat with some briefly gathered loot and return to the shrine of Brigh (the newly minted god of Naval prisons…). Dinvaya clearly doesn’t have the capacity to raise the PC’s from the dead, so their only option is to travel to Chessed.
The Cathedral of Abadar is a most spectacular place. The PC’s were greeted by Monty, Chief Administrator of the Cathedral, who arranged for their raises. He also got to upcharge the PC’s an extra 2,500gp by offering the high priestess of the Cathedral to perform the ceremonies herself. She was most pleased with Monty’s “upcharge”, and awarded him with a percentage of the fee.
The PC’s, near broke, are intact and whole, as they return to finish Hellion once and for all…

Indeed she is, she's also mentioned in the Numeria Campaign Setting, which has been quite useful for adding in extra encounters and sidequests. Up next for my party once they finish book four: The Chapel of Rent Flesh, where they can expect to find a very irate Paajgat looking to make there lives miserable, if they fail to kill her in book four.

Only healing left in the party is a druid so a cleric or similar would be good for the party, since the party wasn't so good to poor ol' Ylrynyr maybe a neutral-ish warlock would serve them right. The dm allows new characters one lvl below who died & since I never missed a session (& everyone else had missed at least a couple) I was a bit ahead anyway. Sigh.

The Gory Details: The heroes saw a pool of fluids and made a pact that everyone would try it. First the Dwarf Fighter drank some and withstood both saves like a champ. The Paladin tried next, failed both saves hard, and rolled a 9 on the Fluids table, and his guts melted into a puddle around his ankles, and died outright from multiple negative stats. Not surprisingly, no one else drank after that.

She was later on resurrected with the scroll promised by the town to the PCs, but... ouch!

Catalyst: x3 Favored Enemy Crits and Bane Arrows.
The fight had been going pretty well. Our overly cheerful and trusting Lava Gnome Pyrokineticist had happily gone inside Birdfood's mansion to talk him into willingly surrendering his position to the Steel Hawks, and almost immediately sprung the pretty obvious trap.
The rest of the party smashed their way through a small horde of orc mooks as the Pyrokineticist tried to hold off Birdfood, his bird-friends and another small mob of orcs.
Rushing ahead to the rescue both Azver and Cristos neglected to find cover from Birdfood's arrows only to discover that while he hadn't been overly deadly to a Gnome buried behind a pile of cover (orcs), he could take cruel advantage of all his suddenly active bonuses against a couple of unsuspecting humans.
A Gravity Bow charged Bane arrow slew the fragile and already wounded sorcerer, and two further arrows including a confirmed crit to the throat of Gorum's Champion (6d6 on dice alone!) were enough to get the 2nd kill of the round, and the campaign.

The Gory Details:
So the four party members (the cleric of Brigh was absent this game's session). Cleaned up on the Aurumvorax on the level below, and although low on resources, have done quite well since first encountering Furkas Xoud on level 1.
I accidentally moved the electrical trap from #25 to the bedroom entrance door. Hilarity ensues, as they fail to look for the trap the first time, and Congar the wizard/investigator/trap remover gets zapped for 47 points of damage (By the Gods, his 16 Con has saved his bacon more than enough). Some infernal healing later, he tries to remove the trap on the door. Poor dice rolling contributes to Congar setting off the trap again, for 52 points of electrical damage (dice were hot Monday evening...). Congar goes unconscious, and the party is able to revive him. They open the door (perception-detect magic, it did not reset yet), and patch the wizard up.
They search the bedroom, and Defender discovers the hidden location of Furkas' spell books. Furkas preps a mirror image (+5), and manifests in the bedroom, next to Defender.
Alexander the Inquisitor (i.e. Helmet full of hate) launches two arrows at the wizard (current buffed AC 28), and hits once for minor damage. The party has no ghost-bane/ghost-touch items or effects. Lambert the Bard begins dancing, and readies to jump the wall.
Furkas casts a 50 ft., 15ft high Wall of Force, splitting Defender from the rest of the party. Currently Alexander has see invis up, and can see the Wall of Force effect.
Defender 5ft. adjusts and power attacks twice, missing, but taking out two images.
Congar casts Fly, and flies over the wall of force, landing about 10 feet behind Defender.
Rd.2
Alexander drops the bow and takes out a grappling hook, and tosses it on top of the wall.
Lambert takes the hook and rope and easily climbs over the wall.
Furkas casts Phantasmal Killer on Defender. Defender thinks of a world without Adamantine weapons and begins to cry horribly (Fails his Will Save). He gets really angry at Furkus for making him think such impure thoughts (Makes his Fort Save).
Defender power attacks and hits once and takes out a mirror image.
Congar uses a wand of magic missiles, but realizes that Furkas somehow recast his Shield spell.
Rd.3
Alexander casts a buff spell on himself (I forgot which it was).
Lambert swings his enchanted scimitar while doing his happy dance, and hits once, removing a mirror image.
Furkas casts Cloudkill.
Congar and Lambert fail their saves, Defender makes his. 4 or 2 points of Con damage.
Defender removes the last of the mirror images, and also hits once for minimal damage.
Congar flies up towards the wizard and casts Dispel Magic, dispelling the Shield spell.
Rd. 4
Alexander casts Delay Poison and begins to climb the wall of force.
Lambert slices at Furkas doing minimal damage.
Furkas casts Solid Fog. Things get stuffy...
Congar and Defender fail their Fort saves, Lambert makes his [4 or 2 point of Con damage from the Cloudkill]
Defender swings at Furkas. His dice are abysmal most of the night...
Congar slams two missiles into Furkas from the wand.
Rd. 5
Alexander casts Delay Poison on Defender.
Lambert slices away, hitting once for minimal damage.
Furkas casts Irradiate.
Congar takes 2 points of Con DRAIN, Lambert takes 1 point.
Congrar makes his Fort save, but Lambert fails his. [2 or 1 point of Con damage]
Defender swings at Furkas, hitting once for minimal damage.
Congar flees the combat, down to 2 Con. Lambert, also low on Con, flees the combat.

Furkas invokes his Miasma Cloud, doing 9 points of negative energy, and healing himself in the process.

Alexander fails the fort save (Cloudkill) and takes 3 points of Con damage. Alexander makes the Fort save (Irradiate) and only takes 1 point of Con DRAIN.

Alexander takes out a scroll of Delay Poison and recasts it.
Defender swings and hits once for minimal damage.
Furkas casts Shout at Alexander and Defender. Alexander makes his save and takes 9 points of damage, Defender fails his save and takes 19 (and is deafened). The Miasma Cloud does 10 points of damage to Alexander and Defender.
Rd. 9
Alexander flees combat, due to low HP.
Defender flees combat, but only makes 15 ft. up the rope.
Furkas casts Web in the area Defender is in, but luckily Defender makes his save.
Rd. 10
Alexander flees the tower (and is out of combat).
Defender makes it over the wall, and takes a few points of falling damage.
Furkas flies over the wall and casts his Close cantrip on the door (The trapped door that technically didn't reset for another round...).
Defender swings in anguish at Furkas. Furkas zaps him with the corrupting touch and kills him outright.
Furkas rummages through his items, taking a brown access card and a Haversack with some batteries in it, then Telekinesis the body out the window.

I had dropped several hints that something big and mean was stomping about, the group had just rested, and with that big open dark cavern it was the perfect place for the rogue with darkvision to set up an ambush. But they, with all 9 Con that they had, walked openly into the cavern, and with top initiative charged the Ettin, trying to hold toe-to-toe with it (despite having a fighter and a barbarian in the party who could have held a front line). The ettin got a full attack with both spiked chains, critted with one of them, and so the rogue was pureed on turn 1 as the rest of the party watched in horror.

The Gory Details:
From the upper level, Birdfood rained death on the party. His attacks were so effective as is that I never bothered using his human-bane arrows. He quickly brought down the cleric (Aden) with one round of attacks. Next round he was able to down the party's half-elf barbarian. The party, meanwhile, had only managed to down two of his hawks. At this point the party realized they needed to flee and started dragging the fallen down the front stairs, but it's hard to outrange a composite longbow...
Birdfood, extremely angry at the loss of two of his birds and seeing the party in retreat, wanted to inflict the same pain upon the party by killing their animal. Despite being much better at killing humans, he began targeting the sorcerer's (sylvan bloodline) animal companion (a small bear). This was also a bit of mercy from a DM who didn't want to wipe the whole party. The bear was tanking the damage well as the party fled and the sorcerer used Vanish on the party's fighter so he could drag the bodies off in relative safety. However... The sorcerer loved his bear and wasn't willing to let him die, so he ordered the animal to fetch a piece of junk far in the distance. With the bear taking a run action and leaving everyone else behind... Birdfood took the only shot he had, at the sorcerer. Oda the Sorcerer died quickly, brought to negative Con with two arrows. The previously downed Cleric bleed-out as no one had the time, magic, or skills to stabilize him fast enough. The downed barbarian, however, did stabilize and later recover.
As a side-note, the party's half-orc fighter had promised to rip the arms off anyone who harmed their cleric and chop the legs off anyone who harmed his half-brother (the barbarian). He made good on this promise when they returned a week later to face Birdfood again. After a bit of interrogation, dismemberment was Birdfood's cause of death as the fighter ripped his arms from his body bare-handed. He now carries around Birdfood's head as a trophy, holding it up and yelling "SCRAP WORTH!" whenever he meets anyone in Scrapwall.

One kill, and one awesome near kill:
Book one, the fight with Hetuath is ROUGH. He is full attacking his way through the party, and just ruining them. The fighter is down, the investigator can't land a blow and is injured, the sorcerer is out of spells. Hetuath drops the cleric to exactly zero hp. Things are teetering towards TPK. The cleric decides to go for it: he'll throw some healing at the sorcerer, and take the damage from acting. His spell provokes an AoO from Hetuath, who crits. The android cleric of Brigh dies on the spot.
The players were genuinely frustrated. Hetuath's DR, immunities, and high AC made him hard to damage. The cleric's death was the turning point – just traumatic and dramatic enough to be cool, but if had gone any further south, it would have stopped being fun. So naturally Sanvil Trett showed up to help put Hetuath down. Sanvil never turned on the players, but still accomplished some of the things he was after. And the players knew something was fishy and haaaaaaated him. But it never came to blows.
Book Two, the fight with Birdfood:
The party just strolled up to the front and called them out. There was a pretense of being mercenaries looking for work, but no one bought it and instead prepped for battle (gravity bow is GO!). The sorcerer cast grease on Birdfood's bow, and the save was failed. The rest of the party tore into the orcs and dispatched them quickly. Birdfood failed a second save to try and pick up the bow. He drew his longsword, and the sorcerer greased him again, save failed again – and reflex is his good save! He was next entangled in a net. Birdfood is having a terrible day. The fighter finally closes on him to end it, and Birdfood made one last entangled, improvised weapon attack with a human bane arrow and crits the human fighter. Fortunately, Birdfood died immediately after, and the fighter was stabilized and healed.

Catalyst: Head shot - boom!
Mallory, ward of Khonnir, 8 years old had spent her entire life in Torch. Finally seeking Khonnir's blessing to leave into the dangerous wilds of Numeria with her new-found friends at the end of 'Fires of Creation', she mused to her friends about how much better she would be in combat if she was able to find a firearm rather than a gun. The forces of fate (ie, the DM) overheard this, and the next day the evil Scrapwall junker known as Birdfood, "found" a magical musket that he thought was a blessing, not knowing that this musket was ultimately placed in his hands by the powers of Good so that Mallory would find it.
Several weeks later, Mallory and her friends began the seige of Hawk Palace. Caught in a firefight from the top of the palace, Mallory shouted to her friends to charge Birdfood's minions while she provided covering fire. Her and Birdfood ended up in a solo-long distance sniper-off.
Sadly Birdfood, on his first attack, rolled two natural 20 (to hit and confirm). Combined with human bane bullets, favoured enemy, and a x4 crit on the musket, took Mallory's head off in one shot. She would have survived had Birdfood just been using a bow. His third shot was also a natural 20, dropping the Ranger's animal companion.
Eventually Birdfood surrended, and begged for his life before Mallory's friends. He was not spared.

35th session of this game after 3.5 years, and this is our first death of a PC who has been with the party since the beginning (we had a temporary PC join during Choking Tower, who died quickly - much less dramatic). The team had brought Sanvil with them since book 1, basically turning him into a reluctant ally and eventually getting him to turn his back on his Technic League past. He was, however, killed by another PC who went crazy, and his body was hidden by the party until they could leave the Valley and get him rezzed (again).
That is, until a brain collector stole his brain. Having returned from a weary fight against the Shipmind, they set out immediately, without resting, to track where his brain had been taken, and headed straight into the Dominion Hive. There, they had a disastrous encounter with the Damaged Annihilator, and Isuma sacrificed herself to heavily damage it and allow them to escape. They spent a rough night in the wild, getting attacked anytime they attempted to rest, before retreating to their cave to regroup and heal.
On a second foray into the Hive, they ran into the Incubator fight and things went nasty fast. Kyontha was downed by a pair of Ugothokras, and one proceeded to eat most of her torso while the rest of the party finished the fight. With no cleric, no rez spells on deck, and no easy way to travel out of the Valley and get her fixed up, she is currently considered down and out for the count, and her player is rolling up a new character (and a cleric, to boot).

While not a total party kill, my entire group went down (and was subsequently captured) by Meyanda at the end of Book 1.

Location: The Reactor Room
Catalyst: A caster on a 10ft tall platform / terribly below average rolls.
The Gory Details: (Having heard that the collector robot was a problem for some groups in the Meyanda fight, I replaced it with two of the half-orc scrappers featured elsewhere on the floor. I also recognized that her stat block is pretty potent if you don't follow the established tactics; ie. using her so-so spells first. So I more or less stuck to the order given.) Smelling a boss room up ahead, the PCs, overconfident after a rush of successes on the last two floors, make no preparations and instead open the door to the Reactor. They squander their surprise round by spreading out and/or buffing, and still get nailed by a sound burst for 6 points of damage with half the party stunned. A Searing Light for almost minimum damage and a lesser confusion both land, but to little effect. Unfortunately, the PCs can't capitalize on this because they're rolling nothing above 10 on almost every roll and are barely able to dent the two scrappers.
Though they briefly put up an obscuring mist and regroup, they have no good way to attack Meyanda up top without climbing the ladders, so they decide to rush for the ladder all at once. Unfortunately, they rush in as a group to the same ladder and all get nailed by a channel negative (for below average damage), dropping one PC and leaving a few others close to unconsciousness. With one scrapper still up and Meyanda switching to her inferno pistol, they never manage to stabilize and get mopped up.
The Tragedy: The fighter got in a great tactical position with his reach weapon to make sure Meyanda would provoke whenever she moved or fired her weapon. But then he proceeded to roll >8 misses in a row and went down to the CR1 scrapper that had him pinned in the corner.

Party tried to open up the Central Processor room and that triggered a hallway fight, with the fighter putting himself in the way protecting all the other party members, the druid got rid of the small fries, while the gunslinger was picking the lock to Central Processor room. Kulgara got in some good hits and the fighter went down, but its victory did not last long as kulgara went down before they could act once more. After they use the salve of second chances and he rolled a male orc once more, and as it was near the full moon he rolled twice and got male orc once more.

Catalyst: SPLOSIONS!
The party had managed to sneak into the compound, and pacify the first basement level, and had just climbed up into the main compound when they got spotted by a gearsman with a rocket launcher. He fired into the room and alerted the rest of the compound to intruders. While the gearsmen were not incredibly dangerous once they closed the distance, the sellswords and battle mages ended up being slightly more efficacious, especially since the party was fighting on two fronts. Ragnal managed to hold off one of the sell swords, and even drop his foe, but the two mages let loose with a pair of fireballs, which briefly ended the dwarf's life. Fortunately, a breath of life from the lore oracle managed to bring him back up, and an aggravated MEAM (Metal Elementalist Android Mage) overkilled the mages with a chain lightning.

Hellion really doesn't take well to being called a false god and having an inhibitor chip shoved up his port. To prove itself mightier than the champion of Iomedae, it started pelting her with plasma shots (the party's Arcanist and Druid got lucky with Lightning Bolt and Arc Grenades and disabled Hellion's scrapyard robot backup, so they didn't need to worry about that).
She charged at the arachnid robot with Kulgara's chainsaw and kept trading blows with the AI, with both getting dangerously low on health. Julia went down to a plasma and claw combo, but managed to stabilize. Until the rest of the party finished Hellion off, killing her in the resulting plasma explosion (The party's druid expended Sarenrae's favor from Aldonard's grave to bring her back to life)

After a stream of easy wandering encounters, and an absolute romp against the Smilers, the party was in high spirits for the attack on Hellion's Stronghold. Helskarg was there to meet them, and when she descended down off her platform, they massively underestimated her. The barbarian ran forwards and took a full attack that brought him well below 0.

Catalyst: Double initiative is busted.
Whoever put together the Hellion encounter was an absolute psychopath. The party, expecting real trouble, was buffed to the gills with defensive abjurations, including Resist Fire and Resist Electricity. But when the Fighter used his magboots to get into melee range with the big boss himself, Hellion's double initiative meant his HP was whittled down FAST. Four claw attacks and a constrict later and he was out of commission. Since he died while hanging upside down from the ceiling with his magboots, the party had to grappling hook his corpse down for future resurrection.

Smilers let party through gate. Only Sting and Eram(Party Oracle/Tank) walk through gate while rest of party hang back. Both fail their reflex and take 10 damage from the gate and are pinned. Smilers get to go before Sting. Half shoot at Eran half shoot at Sting. Last Smiler to go shoots at Sting Rolls 20 to hit. . . Rolls 20 to confirm . . . rolls 7,2,8, and 3 for 20 damage crit while Sting was at 3 hp with a Con of 12.
The party had zero problem with the Smilers once they got through the gate. Sting just got super unlucky. Between the Party Alchemist and Gunslinger the Smilers went down in 2.5 rounds. I feel bad. The party has the Res scroll from book one and are willing to use it but the player of Esik/Sting says meta that the character isn't worth the 12k the scroll is worth and although they like the character thematically they don't feel it is very useful to the party. I like/d the character and hope between sessions they change their mind and let the party use the scroll we shall see. . .

These guys sliced through almost 6 books of the AP like hot knives through butter. The Overlord Robot killed them in two rounds. Technically, Jacquen and the Technician were both still alive, but running for their lives and unlikely to make it when we called a halt, but I'm counting it as a TPK.

I am pretty sure you do get sneak attack, but I'm not positive.
The tocket uses an attack roll to hit, but you use a save to reduce damage. The target of a direct hit does not get a save.
Without other clarification, I would say the direct hit gets sneak attack but not the others in the blast radius.
I'm about to start book 5 so ticket launcher will be more prevalent. Though I am swapping the rockets for different tech guns on some battleguards to reduce the mountain of missile launchers the part will have.

After having a pretty easy time with book 3, book 4 has been biting chunks out of my party. The freeform nature allows them to push into harder sections without leveling up first. Oops.

Catalyst: Stompy stomp goes the damaged Annihilator.
After meeting Isuma, the party becomes determined to help her liberate the bodies of her tribe, to the exclusion of any other adventuring. This takes them right to the Dominion Hive and into a fight against the damaged annihilator robot. Despite some small tweaks to make the fight a little easier (Still APL+3), the barbarian sits one round too long in melee and eats two melee attacks (including one crit) that bring him from half HP to the deep negatives.

Location: Lake around the Dropship
Catalyst: Not prepared for aquatic combat makes for easy prey.
Advanced Lukwatas deal some absurd damage, and this particular one manages to sneak up on the party swimming over to the dropship. The oradin eats an early power attack bite, then another, leaving him low. Without any swim speeds, getting away and repositioning proves difficult. In true self-sacrificial paladin fashion, he chooses not to dispel the shield other he has up on his allies, so when the lukwata lands a critical hit AoO on one of them, he eats another 40 damage and is thrown deep into the red. The party narrowly avoids the lukwata escaping with his body, and they're able to revive him.
