The body is still at the entrance of the secret tunnel.
Elena readies her Tactician (Precise Strike) ability for the moment the creature shows up (+1d6 dmg on flanking) and a swift action to throw a challenge at it.

Elena and Zyl see the entranceway and surrounding bits of wall from the eastern stairwell explode. Elena sees tentacles wrap around the ‘breadcrumb’ lunatics chained to the wall, drain them, and toss ‘em aside like rag dolls…when the beast steps fully into the chamber, it becomes visible to Zyl as well…it is beyond your comprehension…while its assumed primary head spits out a piece of lunatic, the other heads look at the pair and roar…

Zylencia and Elena boldly go right at the evil spawn…unfortunately, the beast is quick and has a long reach and it bites at both as before they get close enough to strike…
Bite (Zyl): 1d20 + 18 ⇒ (3) + 18 = 21
Damage: 1d8 + 8 ⇒ (3) + 8 = 11
Bite (Elena): 1d20 + 18 ⇒ (7) + 18 = 25
Damage: 1d8 + 8 ⇒ (4) + 8 = 12
…and it’s teeth sink into both!
However, the Pathfinders get retribution quickly. While one of Zylencia’s attacks glances off the thick-skinned beast, the other is a mighty stroke, slashing deep into its torso causing it to roar in pain as blue blood splatters over both Pathfinders… (crit confirms due to it being flat-footed)
Elena follows that up with a worthy blow of her own…her magical blade cutting deep into its other side, causing a wicked gash and a slow viscous flow of blood, causing extra damage against this type of foe, as it howls ever louder…

Piotr breathes a heavy sigh of relief after realizing the ceiling and creature didn't come crashing down on him.
Steeling his resolve, he rolls his shoulders and puffs out his chest as he moves into the great hall. He howls a warcry as he strides conifidently towards the beast.
"AAAHHH-WOOOOO!!!
...
"Aww, crap."
He mumbles, noticing Elena and Zylencia are both in front of the creature, blocking his way.
He stops several paces behind the two warrior women and swiftly blesses himself with Milani's divine favor.

Elena spits blood. She's pretty sure she has a broken rib or two. The blood likely helped her slip from the tentacles. "ReAlly NEeD sOME HELP HERE!" her voice weavers a bit with a hint of distress. She doesn't think she can take another beating like that.
She grits her teeth and brings down her sword again with a primal scream hoping to weaken the beast as much as she can.

…and just like that, the spawn of Yog-Sothoth, with shocked eyes, goes down with a whimper…

Elena will hold on the wand then and thank Piotr for the help. She will check on the body and the cultist to see if there was anything indicating what the hell it was and avoid anyone summoning in the future. She's ready to report back to the Mayor. She will ask for the Asylum to be taken over by someone who actually has the patients properly taken care of... she will return the bane sword.

EPILOGUE:
The spawn’s corpse quickly slops away into a foul-smelling mess, leaving behind only a noxious, crusty stain of white powder on the ground. Eerily, its death is marked by a sickening sigh that seems to shudder up from the very earth itself, as if something even deeper under Carrion Hill were mourning the spawn’s death...
As the sigh reverberates, anxious to get out of this eerie asylum and report to the mayor, upon hitting the streets, you hear an even greater noise from the skies above Carrion Hill as wheeling storms of crows descend back to the city’s rooftops, cawing and shrieking and cackling in a cacophony that lasts for hours.
Perhaps this is all a coincidence and nothing more than a final act of a world glad to be rid of such an unclean blight…or perhaps it is a precursor to an increase in activity in the area of the worshipers of the Old Gods. With the tale of the heroes spreading, will it be long before the tunnels under the Carrion Hill are explored again?
In any event, with the spawn slain, the Carrion Hill Horror comes to an end and life begins to return to normal. Mayor Heggry honors his promise to award you, and in the light of their vanquishing of the hidden danger presented by the Keepers of the Oldest, he increases the award by an additional 3,000 gp. Of course, the citizens of Carrion Hill are not the only ones to take note of your triumph. The region has long been haunted by the agents of the Old Ones, and they do not take kindly to meddlers in their ways. They are patient, but they are also vengeful. Man may rule now where they once did, but as the stars draw right, the time of the Old Ones' return draws nigh, and the time for heroes has only just begun.

Aatish takes a sample of the spawn’s remains and is a little intimidated by the crows.
Aatish tries to remain silent as she is not really interested in the praise from some mayor who has never fought in a war before!

When the mission is over she says farewell to the team.
“Aatish will return to Belkzen now! You take care of yourselves and don’t die … except when in battle!!“ she says while leaving.

There might be a way I can acquire the deed to that Elm Way Church, possibly with the help of the Pathfinder society. I'd also like to look into finding someone to properly take over the asylum."
