Every other GM I've played with has declared that moving through allies' spaces provides cover and therefore does not allow AoOs. Also, the cover from the fog should be preventing AoOs. Just sayin'.

I asked our local VL who is also our local rules guru and he says that Michael is correct and when you move through an allies square the ally doesn't provide cover. Total concealment prevents aoos. Partial concealment doesn't. Though it does prevent sneak attacks unless you are an unchained rogue.

Hobgoblin Fighter #2 was not there last round. (He moved there after your turn.) So would Midnar really have stood there doing nothing rather than push into the room where the fighting was?

Yes, absolutely. He's an arcanist. Running face-first into a foggy room is a great way to find said face rearranged, possibly removed.

To be fair my Arcanist was the exact same way. He wouldn't enter a room until the rest of the party had moved around a bit in that room and declared it safe. That is what got him through all of Emerald Spire with no deaths.

Exactly. On top of that, he's a librarian on his first adventure, not big, not strong, but clever.

Fair enough. I'll go with it, though you had no problem running into that foggy room when you thought it wouldn't draw an AoO from the other hobgoblin.

However, hobgoblin fighter #2 is trying to be stealthy, even if he isn't doing well at it. But you have a -1 Perception modifier, so you need to make a Perception check vs DC 4. If you make the check, you can cast the spell, otherwise you won't realize he's there.

tbh, your rules are different to the ones I'm used to. I don't know who is wrong but the whole cover+allies thing, the faerie fire not working like the beam from a lighthouse... my local GMS have ruled differently. Of course my decisions will change if the rules change.
My local GMS always make perception checks themselves, for example, unless you are specifically searching for something.

Obviously, if the enemy isn't unconscious or there are more of them, Midnar's actions stop after drawing the weapon and entering the room.

The hobgoblin that was shouting from the back earlier comes into view weilding a wicked scythe. Selethe swipes at it as he attempts to move past as it tries to move past Selethe.

You botted Midnar last turn. As a White Mage he cam spend one of his reservoir points to convert a spell into a cure spell. Doing that to heal Seleth seems logical since no one else is nearby to heal her.

The hobgoblins quickly fall to the party.

With the defeat of Telda, the attacks by undead along the Sczarni trade route come to an end. You return to find the caravan is still safe.

The rest of the journey proceeds smoothly and the caravan reaches Druma safely.

With Telda defeated, the PCs have managed to eliminate at least one of Isger’s numerous internal threats. Soon thereafter, the Sczarni begin to move goods, people, and secrets through the Chitterwood with renewed fervor.
