The barbarian titan mauler is sketchy and needs clarification. It applies a near-global debuff to the character in exchange for a little extra damage. I think it should stand to be revised into something without the penalty, and more themed around shoving enemies around with forced movement.
Also, the free starting weapon is vague. What are the limitations to the starting weapon? Can it be a legendary magic weapon? This should be tightened up.

For the sluggish 1 I don't think that getting it from multiple source stack so if you get large with later feats you kinda negate the penalty (since you would have it anyway).
For the weapon, I think that having access to a weapon mean lowering martial to simple and exotic to martial if I can extrapolate the ancestry weapon familiarity here. So you would chose any weapon and you have access to it. It is not a free weapon.

Access just means you can buy it.
It also isn't a little extra damage. The weapon itself deals more damage from the size increase (+2 if its already a 1d12 weapon) and your rage bonus is doubled. So a 18 strength raging barbarian with a large greatsword is doing 1d12+4+2+4 damage (1d12+10).
The rage bonus takes +2 to +4, improves to +6 at 3rd, +8 at 7th, +10 at 11th, +12 at 15th and +14 at 19th. Every attack, every round, for each 3 round cycle. And the weapon damage increase is smaller, but constant, raging or not.

Is "zeonsghost" a dev? Because none of that points to larger weapons increasing the damage die, it just says what happens when damage dice are increased. As is, there are no listed rules about how larger or smaller weapons are treated.

I agree with the OP. From what I can tell, at least at first level, and until they add damage for large weapons...

It also increases the creature's natural reach by 5 feet.
These bonuses might very well be specific to the Enlarge spell.
I do think the intent, however, was to have large weapons provide a damage boost and extra reach.
Or is it that the extra reach and damage boost come from the fact that the creature itself is being made one size category larger?
I've been searching but could not find any more information on this subject, did any of the devs actually clarify what having a large weapon means for a Giant Totem Barbarian?

Having a large weapon is a thing you get to have, when you wield it and are raging you get double your normal conditional bonus to damage, which is an effective +2 just like enlarge. You get extra reach from a large weapon via a feat, and via size bumps from other feats. Note, penalties are typed and do not stack, similar to bonuses, this means, while you are at sluggish 1, you notice the downsides of your fatigued round slightly less, and when you enlarge in size you don't gain more sluggishness.
The big advantage for the titan mauler barbarian is how reach interacts with cleave and whirlwind strike, and of course related weapon qualities like sweep and forceful.
And yes, they start out rather pitiful, other than that excellent bonus damage, which is pretty good for a first level character. Most hits on the first playtest arc, if I hit, there was a good chance I would kill it, d(anything)+8 is pretty healthy for a first level character.
