Age: 25+ years. And seriously? Capping at 25? Ouch! (I can double that)
Hours of TV: Less than 1. We used to watch 2-3 hours on Friday nights, but that fell by the wayside, so now it's more like 2-3 hours on a Friday once a month
Books I've read in the last 30 days: This is a hard one, because if you mean, "Have read cover to cover" the answer is 0. If you mean, "Have read parts of", it's 2. If you include Pathfinder books, it's 7.
On a scale of 1 to 5 how much do you enjoy reading?: 4. Trouble is, two things I enjoy more are Pathfinder and socializing, and those eat up all my free time.

Books read in the last 30 days: Um... 5 or 6. When I finished which tend to blur a bit after a week or so.
How much do I enjoy reading: 5. My ideal "good time" is several hours in a comfy chair with a book and as much tea as needed.

That being said, if audiobooks are not included, my answer doesn't change due to the answer choices you've provided. I would think that "a) 0-1, b) 2-4" or, even better "a) 0, b) 1, c) 2-4" would be more revealing, as I assume you are trying to compare time spent reading books verses time spent watching TV. It's easy for someone with very little time to crack a single book at some point in a month. But 2-4 would indicate an actual desire to devote time to reading at the expense of other things, such as TV. It also would probably be more illuminating asking about how many books the repondents finished in the last 30 days.

Books within last 30 days b) 1—4. Though I am not sure if within 30 days that was 1 or 4 (I read three books in short order but it might be just a few days more than 30). This assumes not rechecking game books or peeking just to find some things in other books.

Aight. For those who were wondering about the age thingy; the hypothesis is that as people leave school, they are less likely to read. So the age brackets are
A) Too early to be a very proficient reader, although some may have learned basic reading skills.
B) Gaining reading proficiency, and an age bracket where reading is either greatly enjoyed or forced like homework
C) Middle to High School, where reading becomes a necessity to graduate
D) College where reading is necessary to pass any course, although many don't go and head into the workforce
E) Post School which is usually 25+
Retirees tend to read more I believe. ;)
There was a time a few decades ago when new parents read more as well, when parenting manuals were en vogue. Particularly the "classic" baby book by Dr. Spock (not the pointy eared one).
