I see a lot of people stating that guided hand adds wisdom bonus to hit AND damage. All I can find is in the books is that it apllies to your attack roll. So is it to hit or to hit and dmg? Thanks for the help.

They may be referring to the Mythic Guided Hand feat, or the Guided weapon property.

Guided hand has channel smite as a prereq, so you would need to dip into cleric (or oracle of life or something), also it only applies to the deities favored weapon. (Of course there are deities with unarmed strike as their favored weapon, so it's very doable for a monk).
