Fair questions, but you may not like the answers...(1) no, using the gaze doesn't provoke an AOO. (2) yes, the extra save is due to it actively using its gaze attack this round. (3) Dario was in the room so needed to make the save. given he ninja'ed my post before however I'll cut him some slack and apply his action to Kenji.
Zuriel finds himself lost in the gaze of the construct where he joins Kenji in blissful contemplation of the colours.
From outside, Dolres drives another arrow into the construct.
Saphera steps up and smashes the creature with her sword. There is a horrible cracking sound as she hits it, but it keeps moving.
Dario, quite taken with his success in freeing Saphera does the same to Kenji. slap Did he really have to hit so hard?
The construct returns Saphera's attack, with a vengence.

The profound images cascading across the constructs face become overwhelming for the young inquisitor. The flashes of light and horror filled images send her back to her days in the world wound. It is evident to all that this construct is different from that fought earlier. This ones glass does not have the hardness that the others metal skin provided.

Yes, if you are attacking it, you must cover your eyes (50% miss, no save needed), try to avoid gaze(20% miss, 50% chance of no save required) or attack normally (no miss, save required).

Kenji swings round behind the construct, right through the area of grease [ooc]I'll roll reflex to keep things moving. ref dc13: 1d20 + 3 ⇒ (15) + 3 = 18 He is as agile as a panther and is now poised behind the construct.
Dolres fires an arrow, but this time finds herself out of luck as her allies start to cluster around the odd creature.
Saphera gets ready to swing, but her vision slips into the glass faceplate and she becomes distracted by the lights.
Dario then gives Zuriel a loud clap to try and break the things hold in his mind. Zuriel shakes his head as he comes out of the trance.
The construct turns on Kenji, and attempts to bash him.

You can each attempt a diplomacy check against each of the NPCs one final time...Don't forget +4 if you are silver crusade. They are both former bandits, but they are pretty cowed by your prowess, and maybe thinking about changing their life stylke - so you may wand to decide what you are going to do with them when you get out (which is now, this is just the wrap up. Well done!

Once the group moves to a safe location, Saphera turns to Meleren and Shechera. “Once we meet up with our venture captain, I will be obligated to disclose how we came across one another. If this is going to be a problem for you, please voice your concerns now. Keep in mind, although I will not hold anything back from the venture captain, I will give both of you a positive review for helping us through this.

Conclusion
Once you escape the sublevel, retracing your path
through the upper dungeon and back to the surface
is simple. Initially, Ambrus Valsin is stunned into
silence by the your report, then heaps simultaneous
praise and disapproval on you—he commends you
for surviving a much tougher test than intended,
but criticizes them for not immediately leaving and
reporting in upon discovering the security sublevel. In
the end, though, both the deed and discovery stand out,
and local Pathfinders—many of whom begin in-depth
investigation and cataloging of the hidden complex—
regard the you all with high esteem. In the months to
come, explorers seeking more hidden secrets swarm the
Red Redoubt
