The rolls weren't high enough to find the entrance just like that, but they were close enough not to send you back to the other rooms first.
After going through the room, careful to ignore, but keep away from the brown mold, you find a staircase/shaft hidden beneath the mossy area. The shaft deposits you in an alcove at the far end of an antechamber, free of the riotous growths of fungi and mold that choke the shaft and archives above. The simple, clean lines of the basement’s basalt walls give way here to a disturbing, organic style of architecture that makes it seem like you are walking through the gullet of some monstrous behemoth. At the far end of the hall, a large, ornate arch that looks disconcertingly like the beast’s toothed maw stands in the wall, covered by a heavy black curtain with sickly green light from the chamber beyond shining around the edges. A plain iron box sits on the floor in the middle of the room.

Andrezi looks all around, clearly fascinated by the architecture. "This is both astounding and disturbing. Hopefully none of us will ever have to compare these walls to the real thing..."
Before entering the next area Andrezi will tap himself with his wand of shield

"I wonder what's in the trap," says Azar. "Errr, I mean, the box in the center of the room."
Although he makes light of the very conspicuous box in the center of the room, the green light emanating from the next room worries him more.

Thran puts his finger in front of his lips and points at the curtain before making tip-toe motions with his fingers.
Unless we've given up on the sneak attempt?

Thran still doesn't want to mess up the chance stealth people have and hangs back a bit, ready to sprint forward if or when needed.

The walls and ceiling of this chamber bulge outward, continuing the grotesque architectural style of the antechamber, as if you were standing in the leviathan’s stomach. Queer, glowing nodules protrude from the walls at seemingly random intervals, bathing the room in their sickly green glow. A tall, free-standing, almost organic-looking archway carved from a strange, glowing blue-green stone stands atop a low dais in a wide alcove at the far end of the chamber. The center of the arch is empty, revealing the wall behind it, but the space occasionally flickers with curtains of starry darkness, as if somewhere else lies just beyond the portal. A susurrus of ghostly whispers seem to issue from the gate, their language and meaning unclear, but fraught with a sense of wrongness.
Imrizade Blakros stands on the dais before the arch, connected to a cylinder at the top of the Gate of Beyond by five disgusting, fleshy, fungoid umbilicals attached to her hips, shoulders, and the base of her skull. She is half-Osirian, with long, dark hair and light brown skin, wearing a weathered explorer’s outfit. Her eyes are completely black, showing no iris or whites. At the side of the room, there is a zombie, the undead remains of one of the museum’s unfortunate watchmen. The bodies of four other dead watchmen litter the floor around the dais.

Unsure of what Osborn is going to do...but I'd like to charge the woman with the tentacles sticking out of her body. Can I cut her loose from the cylinder she is connected to?

Thran charges forward, his falchion a blur as he slices it through the air.

In order of preference; Trying to cut her loose -> trying to damage her. If Osborn happens to block my path, moving towards the zombie and hitting that instead

Sorry, i am Australian and i was sleeping since Fjuri's post was close to 11pm here and Thran at 2am.
Judging that the distance to Imrizade is too far Osborn whirls his sling and launches a rock the the zombie.
Sling to hit: 1d20 + 5 ⇒ (20) + 5 = 25
Crit confirm: 1d20 + 5 ⇒ (7) + 5 = 12
Sling Damahe: 1d3 + 1 ⇒ (1) + 1 = 2
Sneak attack: 1d6 ⇒ 5
So that is 7 damage if it is not a critand 9 if it is, i think

Osborn hits the zombie, but the wound closes almost as fast as it was formed. Not completely though.
Thran cuts one of the cords attached to Imrizade. As he does it, she yelps and curses him.
Dam: 1d3 ⇒ 3
She then steps away from him, and starts casting a spell. You notice she's also healed from the damage she took from severing the tentacle.

Andrezi listens and recognizes the spell as one he casts regularly. "Ware! She's trying to make herself larger! Let me put a stop to that!"

Move: Move to within 25 feet of the caster
Standard: Magic missile to disrupt casting.
Hopefully this doesn't hurt her too much, I'm not sure if my magic will be able to hurt the wires.

The cylinder is attached to the arch, which is 15 ft high. Azar might reach it with his long-spear with some effort. Thran however can't and misses completely.
Imrizade takes out a wand. provoking an AoO from Thran and Azar. And shoots a color spray spell at Azar and Osborn. Will DC 12 All the while, she heals some of the damage she took earlier.
