A simple solution would just steal somethings from the gnome description- a tint of olive or gold in the skin, and brightly colored hair and eyes. Green seems like a reasonable choice for 'something is off', although just a tint of it at the edges on otherwise blond hair (since fairies love blondes/blue eyes) might be good too.
Heterochromia doesn't seem that unusual either. Maybe even sectoral heterochromia (where only part of an iris has an off color).

Ok I have to ask the DM about that now just because it sounds hilarious. Btw chose hetero chromatic eyes because dual born suli so yeah...anyway...
First World marks...have to agree with looking at gnomes or taking a look at the half- fey template for ideas...it's would say an odd colored birthmark in a shape often associated with the fey

Getting a skin discoloration whenever you touch cold iron. Not a burn or injury or anything, just discolored for a minute.
Getting characteristics from specific fey could also be interesting. Like getting fish scales in a place or two like a nixie, or maybe barkish skin on your back like a huldra.
Maybe a soft glow that you can suppress like a sprite? Not enough to actually DO anything, obviously, but enough that you can look interesting in certain conditions. Perhaps limit the actual glow to certain parts (eyes, hair, and/or a enigmatic rune)

You could have a literal mark, like a birthmark or tattoo that looks like the "holy symbol" of one of the Eldest, which may mean that that Eldest considers you their kin...or their property...depending on which Eldest that is, the implications could be terrifying!

A nice three pointed knot (triquetra) would work well. Its association with Magda is both portentous and completely ambiguous (and potentially meaningless) due to her association with fate.
Essentially, you can say that you are a destined hero...which means that there is a plot thread that you could explore, or you could leave alone entirely (since you can claim that it was part of another random plot thread).

It's unusual in humans, at least. And I went for the off-color eye being really odd by itself (almost completely white, barely any pupil).
Ms. Pleiades wrote:Issue with heterochromatic eyes is that's the usual mark of "This character is a changeling."Kind of the point, right? And she was abandoned by her tribe as a infant, so it fit the background.

Your blood is some odd substance like white wine, green ichor or pine sap.
You have cloven hooves like a satyr.
Cat eyes. Or eyes like pebbles, gemstones, etc.
Extra fingers, or extra joints on the normal number.
A tail (any type). Unless of course your race usually has a tail, in which case: no tail.
Hair made of feathers. Unless you're a tengu, in which case: humanish hair.
Skin of an unusual colour or texture.
The possibilities are endless.

Your farts smell like candy and flowers.
And here I am, sitting on the toilet while reading this... Thanks for the laugh though
On topic:
You could have actual butterfly wings. Obviously useless, but look pretty (especially when not covered by clothing). They also come in different sizes, from barely noticeable to unrealistically large.
