Non combat / non crafting... social professions, with skill trees and that produce buffs / debuffs to the adventuring classes.
Yes I know that is a throwback to SWG, but that is what made that sandbox MMO uniquely different.
These professions could be tied to the settlement's allowable alignments, to add more flavor for different types of communities.
All of these would have skills and grant Buffs (to self or others) or Debuffs or negative effects (to self or others).

Dear heavens, I miss SWG and its battle fatigue. I think that would actually be a great feature for PFO, it increases meaningful interaction, provides a good hub, and gives people a real reason to build roadside inns.

I guess a couple of things come to mind:
(1) Having PFO "feel" like a medieval fantasy environment with all the sights and sounds you would expect to see and hear. Eventually that means NPCs working away in a settlement, numerous non-standard professions available (Bluddwolf mentions many above) including also vintner, brewer, farmer, glassblower, animal breeder, author, artist, trapper....
(2) Being able to alter the environment like digging a hole, then a tunnel, then a mine, and later a complex or city!
(3) Music like in LotRO. They have a great music system, and it would be awesome to have a way to synchronize several different musicians to have real musical groups playing in taverns, inns, and playhouses.
(4) Plays and theatrical events put on by characters with a similar toolkit allowing the actors and actresses to emote fully and voice a full production (think Grand Ol' Opry in the River Kingdoms).
(5) PFO play area expanding to include some grand places outside the River Kingdoms like Highhelm (Dwarves), Celwynvian (Elves), or Brastlewark (Gnomes).
That's enough stuff I'd love to see....someday.

Some sort of a cosmic karma pool to balance alignments. So if the goal is to have an even distribution of people in the 9 alignment boxes (~11% in each box), then if one box gets more than its share the people with that alignment get debuffed.
So if I'm CE and the CE population is at 20%, then I have an incentive to either move to a different alignment or to drive other CE into a different alignment. Same with LG, LE, etc. All alignments would be prone to purges and holy wars.

That made me laugh a little, but making defeat comical and less deathlike might be ok. Just imagining when a player is defeated, X's appear over the eyes; the (PC) body vanishes before it can fall over; and a loot bag appears in its place. Deathless pvp.

Well, it wouldn't quite hold up to the Pathfinder mystique, but it would be pretty darn cool. Confetti and candy and fireworks! Sounds good!

I can't say it would make the game radically awesome, but it is something I think has real value and will not be in the game - being able to drop items on the ground.
I understand the issue with it from a game design perspective, but having played years in UO, and having used the ability for a wide range of purposes, I'll miss likely never seeing it implemented in a game again.

I think this would be a wonderful idea. Pathfinder has a card deck similar to Tarot reading it called a Harrow Deck It is used not only to tell fortunes but can be used for gambling as well although that is frown upon by Harrow readers.

You know, if the play it super euphemistically, they could totally fit in a Courtesan profession. Describe it as "the courtesan spends time with his/her client for money" or something like that. No need to be explicit.
"Nice evening at the client's home". So, a bit of snacks, a movie, a turn at the Xbox?

How about a real language system?
Can't speak / understand Elvish, then their chat in channel comes as a jumble of letters. Want to unlock that understanding learn it. At each level of understanding, it unlocks more complex words.
If an Elf knows he is speaking to a Human, with limited language skill, the Elf would have to write in simple language to still get point across.

A REAL underdark expansion, including PC races and new monsters."Underdark" is a Toril thing, and I'm sick of the damned Forgotten Realms bleeding into every other world. On Golarion there are the Darklands.
We'll call our characters "Pinatas" instead of "Toons".
Sierra's The Realm and Disney's Toontown have toons in a literal sense, and the distorted proportions of WoW graphics could make them toons too (human males have freakish Popeye arms after all). Maybe the Final Fantasy MMOs have 'toons', since their graphics are distorted in the anime style. I hope PFO graphics won't be cartoonish, and saying 'toon' should flag you as heinous.
Things that I'd love but doubt I'll see:
1. A system for creating our own spells. If they don't start with a construction system and build the initial spells with it, any construction system they try to retrofit is likely to be too prone to weird loopholes, or too limited to be worth the time.
2. Flight. They've said it's not in the Managed Expectations Product, and that likely means they'll make certain assumptions from two-dimensional thinking. If areas are designed with the assumption that you can't get on top of walls, buildings, trees, etc. then the later addition of flight would break the game.

Interestingly enough though, more than a few MUSHES (assuming you're old enough to remember the days when MMO meant a character line interface) would have a process in which a player can become a WIZARD which would be essentially godlike status in said realm.

I'm not sure the existance of prostitutes, nor the mere mention of sex brings the rating up past the PG13 mark. The kingmaker campaign had brothels on the list of buildings you can make, the adventure carnival of tears had a peep show etc... The official line is most likely explicit details being described in public readable communications (or private coms to people who do not want them) and nudity. Noting the existance of sex, and spelling out details, are 2 different things.
admitted, paizo will certainly have to be cautious, because the very nature of players, you give em an inch, and they will assume they are given free reign to take it 20 miles.

Monster races and beastial characters. In fantasy settings I like to play non-human characters or scarred character along the lines of Freddy kruger. I am yet to find a game that let me do so yet though. The wow orcs were to cartoony to ever be menacing, age of conan made it near impossible to look like anything but a standard human and in SWTOR your a human with different colors and mabybe some facial features. I'd give my right toe for the chance to play a a gnoll or a half orc, not looking like he is a human but with bad teeth. But something more beastly along the lines of this. I however doubt I shall. Having been disapointed one to many times.

I see someone plays Mortal Online. Their half-orcs can look anything from mostly human to mostly orc, you can play them as canibals, and they make great fighters. Really good game if you want to be a half orc. Too bad most of the other races are underpowered by comparison. :P

They'll probably still avoid it. US culture is fundamentally broken in that way, when casual violence is considered acceptable game material while the same doesn't apply to simple nudity, let alone references to sex.
