Yan looks with horror at the body. I'm guessing flesh warping, but someone down here is doing it recently. Are there any checks we can make to investigate?

While the use here is clearly more recent than some other bits of dungeon, the corpse has likely been here for months.
Interestingly enough if you pull a chain attached to a slab, the slab folds down, dumping any contents down a 40 foot or so shaft into a room below.
It does appear that fleshwarphing experiments were once performed here.

Sendoski grimaces at the chute.
"Was that how they disposed of any failed experiments? I'm kind of curious what's down there."
Sendoski casts light on some debris and drops it down the chute. Is it possible to see down the shaft?
No other doors in this room? Back the way we came? I assume we don't want to go down the chute.

Yan grimaces, hoping Sendoski wouldn’t do exactly what he did. He steps near his friend and looks into the abyss, hoping nothing looks back.

The debris falls down. It's hard to see much through the shaft but they do seem to drop into a single room into piles of suspiciously clean bones and bits of rusted metal. There's no movement or response however.
The shafts are jagged and rough enough that if you wanted to you could probably climb down one of these if you were trained. DC 15
No other doors from here.

Scarred One looks down the shaft "Clean bones huh? So something is feeding down there, lets hope that with lack of new meat it did not last." Right?

The room beyond the door features rusted iron chains hanging from the ceiling of this room, each ending in an oversized manacle.
The octagonal eastern end of the room contains a circular pit covered by a rusted iron grate, it does not look safe to stand on.
Torn bolts and scrap metal dangle from the ceiling above the pit. Piles of scrap fill the four alcoves that open off the wide hall that constitutes the room’s west end.
A tall blue creature similiar to one a level above, wrapped in chains looks your direction, her gaze lingering on Oorgatch's chain whip before she stage whispers "Pain" and lunges in your direction.

Whether or not Yan Grabs the Edge, the rusty winch above crashes down atop him, dealing bludgeoning damage: 3d10 ⇒ (9, 6, 10) = 25 and on a fail or worse dislodges your grip (DC 24 basic Reflex save). (You can try and grab the edge again.)

Yan falls about 40 feet total into a rust and grime filled octagonal chamber. A wide hallway leads to the south where an acrid smell not entirely unlike beer wafts from. The ceiling is about 15 feet up (then a 25 ft shaft). Other than the debris from the rusty grate and winch and splattered bits of Yan, the room is empty.

Yan sighs. He stands, sheathes his weapon, and lays on hands himself. +18 HP
He looks at the wall and flexes his fingers, getting ready for a long climb. Is the wall rough or smooth?

”Oh, no. ” says Oorgatch as Yan falls, still not understanding why he is being ignored so fully.
Trying to finish things quickly, he stabs again with his finisher.

Opening the door reveals a room full of splintered furniture and rusted scraps of metal. Three cells line the south wall, their bars only a few inches apart. Each cell contains a heap of rotted flesh and shattered bone.
The locks on the doors have long since seized shut, but could be forced open or picked. DC 20 either way.
