Rectitude's mind blows a hole though a five-hundred-year-old old almanac of the Taldan hinterlands.
The World's Most Interesting GM
To be clear, Rectitude destroyed an old (possibly one of a kind) book, and not the voidworm... who he missed. Although to be honest it was probably a book that Rectitude would not have like very much, so his response might still be warranted.

Jezu scrambles on top of the re-dead bird, trying to get a line of sight to the worm.
I think that would use more than a move action, so jezu can't attack
Tofang Siyaah

Tofang shoots at the flimsy light being although there is cover.
Gun, touch attack, pbs, cover: 1d20 + 6 + 1 - 4 ⇒ (7) + 6 + 1 - 4 = 10
Gun damage, pbs: 1d6 + 1 ⇒ (2) + 1 = 3

That's going to be tough considering it is about 6' down from the top of the bookshelves, which Jezu is is not quite even with (figure the dead bird gives Jezu about a meter more in height so his ankles are still in the water).
Please disregard post.
Actually, now that I look at the map, there's no line of sight either (bookcases). My bad. Did you want to take more move?
Tofang shoots at the flimsy light being although there is cover.
Fentar's got a name you know!
Tofang's bullet puts a hole in the stone column beside the bloodrager.
Fentar swings out, but the worm avoids the blow. The thing makes a giggling noise.
Derkhan... Wait a minute. Has anyone seen Derkhan!?

Conditions Deep boggy floor space. With the only light (other than the party's) coming from the room above through a hole in the ceiling 15 feet above the floor to the south much of the room is dark or dim. The voidworm has a blur-effect making it harder to hit.

Thanks for the deep dive! So, Kolris can parry as many times as he has AoOs, but can only riposte once per round?
Also, if you could list how many Panache points Kolris has somewhere obvious that would be cool.
All of Kolris' expendable resources are tracked under the Resources spoiler at the top of his profile.
The elf begins swimming around the bookcase stacks, looking for a better angle of attack.

Rectitude sloshes in the water a bit trying to get a bead on the voidworm.
The voidworm seems to draw a bead on him. It flies up over the bookcase (AoO Fentar) comes down behind the plant man. It opens its cute toothless mouth **chomp!** but can't gum through Rectitude's bark, which is worse than his bite. (AoO Rectitude)

And panache points. Each parry costs one point to perform, and you have to have at least one point left in your pool to then perform a riposte (though you don't spend them to riposte).
Rectitude
|
No AoO from me. I'm not carrying a weapon right now.
Rectitude narrowly escapes the voidworm's bite. He shields his body with sunrod and clenched fist. Standard action to Total Defense. AC 24. Move action which provokes.
Derkhan Ashgale

Swim: 1d20 - 1 ⇒ (19) - 1 = 18
Finally getting his feet sorted out, Derkhan kicks through the water towards the voidworm.

It's tiny so Rectitude does not provoke either.

The voidworm liquid form begins to reform as it dives into the water at Derkhan. This constitutes a 5' float, but because it is into Derkhan's space he may, if able, take an AoO.

"Blasted. Thing." Kolris continues swimming closer, finally getting next to the voidworm, but unable to strike yet for the time it took to swim.
Double move (monkey fish): 15'
Kolris only attempts to parry the tail slap attack
Parry, +1 rapier, flank: 1d20 + 9 + 2 ⇒ (15) + 9 + 2 = 26 1 panache point and 1 AoO if used
Riposte, +1 rapier, flank: 1d20 + 9 + 2 ⇒ (16) + 9 + 2 = 27 if parry successful
Miss chance: 1d100 ⇒ 56
Damage, P: 1d6 + 3 ⇒ (1) + 3 = 4
Kolris is in the water with a swim speed and a piercing weapon, so I believe that no penalties apply to his attacks

Now what?

Swim: 1d20 - 1 ⇒ (10) - 1 = 9
Swim: 1d20 - 1 ⇒ (11) - 1 = 10
Climb: 1d20 - 1 ⇒ (15) - 1 = 14
Derkhan surfaces, swims to the nearest bookshelf, and scrabbles up the shelves.
"Right. Where were we?"
The World's Most Interesting GM
The bird is pretty much as you see him (sans head of course). The protean seems to have transformed into a power like the sort you might add to water and then stir to make fruit punch. Alas, the water in the library is sea water.
However there are always those two scroll cases the party found earlier.

”Let us climb up to some place that is dry and open the scroll cases to see what is in there!“
Kolris Isimlothe

Kolris hands the scroll case he found up to Derkhan.
The World's Most Interesting GM
Each of the scroll cases contains a single weathered bamboo scroll covered in a magic-user's arcane writing.
Kolris Isimlothe

Knowledge (engineering): 1d20 + 7 ⇒ (20) + 7 = 27
Kolris takes a good look at the shelf Derkhan is on, and makes sure that it will bear his weight as well. Assuming that's the case, he climbs up beside the halfling and takes a look at the scrolls himself.
Activating read magic as a spell-like ability.
Spellcraft, free inspiration: 1d20 + 9 + 1d6 ⇒ (10) + 9 + (5) = 24
Spellcraft, free inspiration: 1d20 + 9 + 1d6 ⇒ (17) + 9 + (2) = 28
The World's Most Interesting GM
The shelves can indeed hold Kolris. The scrolls are scrolls of magic circle against chaos and slow in bamboo form.
The World's Most Interesting GM
As halflings tends to weigh about 35 lbs, before equipment and he has a mithril shirt on (+5 lbs) for armor, I think we can safely put Derkhan (and the gnome, and wayang) in the 36-130ish lbs category--probably on the low end. That probably goes for Jezu (who can swim or ride), and Tofang (who probably can't swim). So going as close as we can to marching order lets say Derkhan hitches a ride on Fentar, and Tofang climbs on Rectitude. Jezu can swim or perhaps piggyback on Kolris.
With half the party literally underwater, the party proceeds into a maze of narrow passages crammed with half-sunken shelves, and piles of books, and paper with the bright light of Derkhan's and Rectitude's sunrods to guide them through the meandering passages and vaulted chambers. It's slow going. (Medium loads or swimming slowly through a deep bog.)
The Cartographium, as one might expect, is a large flooded chamber where the water’s surface is carpeted with maps made of silk, vellum, birch bark, and sundry other materials.

Kolris swims into the room and begins looking for scraps of their poem, or anything else of interest.
