Emrael moves toward the nearest worm 5' as she begins the casting of a spell dragon's breath defensively. With a roar, her form appears to be overlaid with that of a huge gold dragon and a gout of superheated steam emerges from her mouth to engulf the head of the nearest worm and strike its neighbor as well. (I'm going with the assumption that they are not truly lined up so pretty that I can use a line effect against all of them.)
Before the steam has dissipated, the woman's katana flashes with power and begins to hew and carve at the closest worm.
Spell Combat allows me to cast a spell while performing a full attack, at a -2 penalty to attack. She has to wield her katana in one hand during these attacks.
Casting defensively for a 4th-level spell has a Concentration DC 23; her Concentration check is 1d20 + 25.
Haste gives her a +1 to attack and a bonus attack.
She will apply a point of her arcane pool to grant her attacks a bonus equal to her Int Mod (+7) for one round and another point to make her katana a +5 Keen Flaming Burst katana.
She will employ power attack for a -3 to hit, +6 to damage and combat expertise and web of steel for an AC of 43.

With a shake of his watery head, he plunges forward. Carrying Claw and Satari in one fluid arm, he can only use the other to attack, but Pip's charm of speed still allows him to get in two brutally swift attacks as he hammers at the nearest worm with Emreal,...
BTW, reminder that in Elemental Fury form, our little elf druid is Huge sized. :)

A pair of bladed hands emerge from Elgan's watery cocoon, plunging into the head of the nearest worm.

The monk floats weightless inside Elgan. Waves of violence ripple around him, tides of combat and bloodshed and danger carried on the tide. His eyes open, perhaps revealing an ember of green fire, and his body jacknifes in a swirl of bubbles.
When the bubbles clear he is gone, flicking into being next to one of the abyssal worms. His fist lashes out, leaving a smoking crater in the worm's flesh crawling with emerald fire. Flicker. He reappears on its other side, and a fist carves green ruin. Flicker. He reappears some way down its length, lashing out again and again. Flicker, flicker, flicker.
Abundant step. Flurry of Blows. Dimensional Savant. Power Attack. Dragon Stance. Flanking with self. Ki pool for extra strike. Attacks are adamantine and lawful.

Swarn 2 attacks Elgan. He takes 6d6 ⇒ (1, 5, 2, 6, 5, 5) = 24 hp and has to roll dc 21 fort or take 1d4 ⇒ 4 strength damage from poison.
Swarm three swims up towards Altai.
Worm 1 bites Satari. He takes 4d8 + 18 ⇒ (7, 8, 3, 5) + 18 = 41 It grabs him in his mouth; if cmb 51 beats Satari's cmd.
It then stings Emrael;
she takes 4d6 + 18 ⇒ (1, 6, 1, 2) + 18 = 28 and dc 27 fort save or lose 1d4 ⇒ 3 strength.
Worm 2 bites Altai. He takes 4d8 + 18 ⇒ (8, 3, 1, 2) + 18 = 32. It grabs him in his mouth; if cmb 49 beats Altai's cmd.
It then stings Claw; does 4d6 + 18 ⇒ (3, 1, 2, 5) + 18 = 29 before Claw's resistances, and....I reckon Claw's immune to poison.
Worm 3 bites Claw. He takes 4d8 + 18 ⇒ (3, 1, 7, 4) + 18 = 33 before resistances, and it grabs him if cmb 39 beats Claw's cmd.
It stings Claw and misses.
Group's up,....Claw, Satari, and Altai might be about to get swallowed.

I'm getting the heck out of Dodge using my Dimensional step ability to move to Y40. From there, I cast Cone of cold, which I Empower with my metamagic rod. It hits Worms 2 and 3, as well as Swarms 2 and 3 (plus Claw, but he doesn't give a crap). Damage 15d6=58x1,5=87 cold. The swarms take a further 50% extra damage: 87x1,5=131 cold. Reflex 25 for half damage.
