Ropey strands of sickly fungus stretch across this rough-hewn chamber’s walls. Vine-like growths hang down from the ceiling, some ascending through a ten-foot-wide aperture more than forty feet above. DC 10 climb check to ascend, but most of you have the ability to fly.

"I'll climb as long as someone can cover me."

Dervak has a +12 Climb modifier with Heroism so makes this on a 1. He actually does lose his Dex bonus while climbing, one of the few times he can be Sneak Attacked, so I don't love the idea. He could cast Fly if needed but his 3rd-level spell slots are very limited.

Kaklatath waits as the rest of the party ascends and then easily starts climbing up the fungus.

Climb take 10: 10 + 10 = 20

The ease at which she climbs suggests that she’s not as incapable as she has made herself out to be. Obvious statement is obvious.

Dervak climbs through the fissure in the fungus. Here the fungal growths are even denser, forming a thick mat of alien, organic matter whose many projections make for treacherous footing. As Dervak looks around he sees a number of small birds staring back at him silently; they all show signs of the same corruption you saw in the chambers below, misshapen fungal growths sprout from their bodies.

They take flight, massing in a twisting undulating swarm, swirling ominously closer.

You've encountered diminutive swarms before.

Everyone can get into the chamber with a single move action, Dervak is already in the chamber. The floor is difficult terrain, fly checks are at -5 circumstance due to the fungus blocking your way.

Religion DC 17:

The translucent image of a woman with silver voids instead of eyes descends into the chamber. She wears ancient Ninshabur garb and holds a bastard sword in one hand. Ghostly blue flames dance around her. She speaks an arcane word and the sword drips with all too real acid.

Fort DC 20: 1d20 + 13 + 3 ⇒ (12) + 13 + 3 = 28

Dervak will use Barroom Brawler to pick up Ghostslayer as a move action, activate the feat as a swift action, then Dex-rage casting Bladed Dash as a free action, and finally strike as a standard action. Using Fighting Defensively.

The swift action to use Ghostslayer prevents using Arcane Strike, so his damage drops by a bit, but because the Furious Falchion is +3 when raging, it will count as Ghost Touch.

Bladed Dash Attack, Heroism, Cha bonus, Defensively: 1d20 + 20 + 3 + 4 - 4 ⇒ (12) + 20 + 3 + 4 - 4 = 35

The woman strikes Dervak through his armor and magical protections in a perfect strike. The blade cuts through his chest that burns his flesh and spirit.

I think that knocks Dervak unconscious.

Yep, 37 damage from Fire Shield plus 66 from the strike is 103 damage; he was at 96 HP after the gaze, so is now at -7.
His 28 AC vs incorporeal touch really is fairly good if she attacks at a +12, but the dice were against him.
I'm actually not too worried about getting crit. Crits against him only confirm on a 20 if he is raging, and even then he has Defiant Luck and Defy Death on top of that. But that's still a ton of damage. I don't think anyone has Divine Interference?
