One of the southern doors opens and a trio of men dressed in loose fitting clothing enter the hallway. They have horned brows, clawed fingers that clutch simple wands, mouths full of worming tentacles and crooked goatish legs with cloven hooves. They are Denizens of Leng, you know that their bite makes the flesh it touches permanently numb 1d6 dex drain, 19 fort down to 1 dex, they have disturbing anatomy 50% crits and sneak attacks turn to normal attack and fast healing 5, and are resistant to magic SR 19.
Kat slashes her spear deftly at each of them but one manages to move past her without molestation. They take aim with their wands and three blasts of light shoot forward towards Kat. She senses the first one coming and moves out of the way just in time, but the other two cut into the half-ork searing into her flesh. 51 damage, if I'm not mistaken.

Kat was absolutely not expecting to get blasted by three fire rays right off the bat, so blocking even one of them was a miracle that she could not be more thankful for.
She quickly downs an extract Resist Energy, choosing fire. At her level, it'll be 20 fire resist then takes a step forward, hoping to get a hit or two in when they try to blast her again. "Just gimme a bit of a heal and I'll be good, if you don't mind, Ms. Mara."
While she's at it, she studies one of the leng denizens Studying blue.

The fort save is so close that it's worth a double look at the math. If Dervak did save it is only 1 dex drain.

Wow, that is quite an unfortunate series of rolls. 20/18/16/20 to hit Dervak for 15 damage (despite his 32 AC), and then a 2 on his save.

Dervak and Kat cuts down Blue and he starts to evaporate into nothing leaving only his clothing behind. The other two look quite comfortable in the stinking cloud as they disappear into the thick noxious mists. Everyone but Dervak can hear their claws scrape against the floor.
They appear to be immune to the stinking cloud. They use the cover provided by the cloud to enter stealth but largely fail against the party's superior perception. Kat would get an AoO with her spear vs Red, with a 20% miss chance. After which the party is up.

Dervak moves back, ends his rage, and uses Total Defense. There is no telling when this fog will end and the enemies re-emerge, so no use exhausting himself (i.e. using up all the Rage rounds).
Was raging for 3 rounds, so Fatigued for 6. Dex is now effectively 5 with the Drain and the Fatigued penalty. AC drops by 6 (-3 luck, -3 Dex) and saves drop by 3 (-3 luck) with an additional -3 to Reflex.
AC is 29 with Total Defense.

Yep action to you. Kat could make this deduction. They're hiding in the cloud to let their fast healing work. But they also don't want to abandon their wands so they're creeping towards them carefully. Kat can see them but they can't see her, but they probably can hear Kat since she is not being particularly quiet. So Kat grabbed the wands and then saw them approach.

Drawing and Sheathing weapons and weapon-like objects: can be done as a part of any move action so long as character has +1 BAB.
You could have stowed your spear as a part of the move action to pick up one of the wands. Similarly you can redraw the spear as a part of one of the actions to stow the wands. What I'm seeing in my mind is Kat stooping for a second with her spear cradled by her arm and side and in one motion slotting them in her belt or backpack or whatever, then readying the spear.
