The simplest one I make is changing the size category of a creature. No, I don't mean adding the Giant/Young Template, nor the Monster Advancement rules for increasing size. I literally just said: You know what, I think that real world animal is the wrong size category. Let me fix that.
The most complex change would probably be that time I made Mogaru a Guardian Spirit. For the record though, I told my players I was gonna do it, so they knew what they were getting into.
Well, there also was the time I made Mogaru (I know, I have a problem) an Imperial Foo Creature. I didn't get to use him, since he is good aligned thanks to Foo Creature, and I rarely ever get to run evil campaigns (so running one that lasts till epic level is pretty much impossible for me), but man, he would have been so awesome.

Complex?
-Dragons:
I don't work with the color-coding they go by, I treat each dragon as an npc and as such their color mean nothing about their element, alignment or abilities.
I had a black dragon breathing lightning and a somewhat cold and sadistic golden dragon and more. The color is more of where they preferer to live at and who their parents were (but as different color dragons can mate this can lead to a torque dragon being an offspring of a blue and green or the infamous pink dragon etc).
A white dragon that is born with a fire element in the arctics will most likely look for a new home like any other npc who would find his birthplace overbearing.

That one was of course more for introducing colorful NPC and stories about them then mere state blocks corrections.

3.5 had the Draconomicon which allowed for Dragons to use breath weapons not normal to their color. It was a big hit at Gencon when they introduced the colossal Red Dragon Miniature.

playing NPC Mr. Shiney in DnD3.5... then I had a NPC Revenant Ranger travel with the party as their guide as he could always find the BBEG... of course I ran NPC Baba Yaga but she's demi-god/mythic status. A Penanggalen Pyrohydra? Beheaded(shrunken head) with Monk arch Drunken Master7 and an array of barrels of liquor?
A few bugs: Humumorsus Giant Beetle w burrow 5ft with 5 natl atks sequences, the bite had Imp grapple and Tch of Fatigue. The Vermiseffodi Helminth Swarm that eats undead (in Homebrew forum). Aquanae Apinae which are underwater bees of 7 types (2-7HD) with different roles in the hive.
