Just going through this so far.
One question.
B4: The Fallen Dragon imparts the PC with a fraction of the Dragon's Power which functions Identically to a Mk 2 Personal Upgrade (Mk 3 in Tier 13-14)
Does this happen IN ADDITION to the Personal Upgrade the PC already has? Can it upgrade a Mk 1 to a Mk 2 (or Mk 3)?

Here is what I'm wondering about same weapon: the in adventure sidebar lists "When wielded by a lawful good creature, the blade deals an additional 1d10 S & P damage and, once per day as a swift action, the wielder can gain Spell Resistance 19 for 10 rounds." but chronicle only lists the "The corroded vengeance is a unique 14th-level uncategorized two-handed advanced melee weapon that deals 5d10 S & P damage and has the dispelling and holy weapon fusions. It has the analog weapon trait and 1 bulk, and is worth 71,500 credits." part as its not part of the description.
So uh, what do I tell players? Do I tell them their PC who buys it also has the LG only bonus since its not listed in the chronicle and not like you can look up scenario unique item from nethys? :'D
Also the personal upgrade forgets to mention if it only lasts the module. I'd assume it does because no scenario gives free items without boons, so I assume you "return" the upgrade at end of adventure like other equipment?

I think the sword you get during the scenario is just not the same as what you get on the chronicle.
As for the upgrade: a Mk2 personal upgrade at this tier is kinda... well didn't you buy that six levels ago already? And you can have only one.
James ran it for us assuming that it CAN be used even if you already have one, but that it's only for that scenario. And that's how I'd also rule it when I run this.

With regards to the stair fight, how long is the staircase? The maps only show the rooms of the level, but the encounter suggests that it takes place, between the levels. And then the "creature" is attacking "above them"

On sidenote, noticed small error: Gelugons in scenario are CR 13, but 2/4 gelugon encounters respectively list their cr as "15" and "17"(since 2 enemies is CR + 2 and 4 is CR + 4)
This is relevant to note that they are still only CR 13 foes even though encounter cr is 15 and 17 for abilities like the trick attack and such but statblock for both tiers lists their CR as if they were 15 and 17 even though that is obviously not the case as then encounter cr(and stats) would be even higher
(there is similar error with orocoran horde which is CR 15 even on higher tier not 17)

My interpretation is that the staircase forms a full loop around the outside of the tower (agreed, would prefer to have that shown on the map). If each floor is roughly 30-ish feet above the last, it translates into a fairly easy climb (rising roughly 1 foot for every 8 feet traveled, by my napkin math). And yes, the horde is clinging to the walls above them.

B3 mentions creatures that fail a reflex save and fall from the staircase taking 10d6 damage from a 100 foot fall. Shouldn't this be 20d6, in high gravity? And does that mean that the second fall, onto the spikes would become 30d6 if it happens?
Does seem like that should be the case, though just to be maximally kind to the PCs I might only double 10d6 of the second fall, not the +5d6 from the spikes.

If anyone's interested, I made a super rough map for B3 in MSPaint using clipart from the internet (aka why yes, I now have "tile flesh wall png" in my search history :D) It's a sideways view map, rather than the standard top-down view, but I figured that makes more sense, since this is more of a "vertical" fight, than a "horizontal" fight (if that makes sense.)

Was wondering the exact same thing. It would seem to be that if it says it functions exactly like a mk 3 personal upgrade, you can't have more than one. That's how I played it.
On a success, tendrils shoot from the dragon into
the PC and impart them with a fraction of the dragon’s power,

Yeah, in both the 3-20 game I played in and GM'd, we went with what CptJames did: you get a bonus Personal Upgrade you can apply to any stat that doesn't have an upgrade already, OR upgrade an existing Mk 1 to a Mk 2 (for subtier 11-12, of course: add +1 if in 13-14.) Either way, it presumably only lasts until the end of the scenario (like the bonus die of damage for LG characters wielding Corroded Vengeance.)

How many Hellknight fire support crew are there in that final battle?
The word/description talks about 3, but the stat blocks and encounter summary each say four.
It makes a bit of a difference.
Actually given how badly pummelled my group is, I'll probably run it as a three person crew, but it would be nice if that had been picked up in the editing.
