And with my added emphasis, I see the flaw in asking the question; almost all bullying that is prosecuted will be a pattern of behavior that covers every sphere, and the online component need not be sufficient by itself so long as it contributes to the overall pattern.

The exact form of the media is not relevant. Any electronic media based harassment is considered cyber bullying. Then when the component of school aged children, teenagers or even young adults is thrown in, and also the victim being known in RL by the perpetrators, there have been prosecutions.
I can guarantee you, if a teen commits suicide over bullying in a video game by his / her classmates, there would be criminal prosecutions.
My niece was cyber bullied by a classmate, and he was expelled from the school permanently. There was also a restraining order placed on him, and his parents. They ultimately had to sell their home and move to a nearby town, and attend school there.
