Junky Monkey: Using big six potions before the most encounters in a row. Potions such as Owl's Wisdom or Fox's Cunning. Fox's Cunning isn't even in the core rulebook.

Not What it Looks Like: Your character gains a reputation diametrically opposite of their actual nature/alignment.
How I got it -- Playing Shadowrun, my character is a Sniper, covering the rest of the team on an infiltration mission one rainy day. The team ends up triggering an alarm, causing the Mission "Boss", a tough combat mage who was out at the time, to come racing back.
My character sees him coming down the street on a motorcycle, and decides to buy her team some time. So she takes a shot at him. I get a ridiculous amount of successes, and she hits him right in the throat. He glitches the resulting piloting skill roll(70mph+rain+shot in the throat) and skids underneath a truck, where he cracks his head open on the vehicle's driveshaft. Blood flows down the wet streets. With the Boss suddenly dead, the team takes case of the remaining guards, finishes the run, and we book it.
Surveillance footage of the shot goes viral, with many Shadowrunners saying my character had to have been a cold-blooded killer to make that shot, and used a tranq dart just to show that he/she could.
Yes, I said Tranq dart. My character is a kind of a pacifist, and she almost exclusively uses Tranquilizer, Gel, and Shock rounds unless absolutely necessary(or sufficiently pissed).

Burnt out so young Lose a character to an oversized fireball within 5 minutes of the campaign beginning.

How much xp for the trees? Alter a major feature of a continent with an oversized fireball within 5 minutes of the campaign starting.
Surpassing expectations Unexpectedly have a majority of the party survive the first session despite all odds.

All alliterative actions are appreciated applicably. Attacks aimed at alliteration actively anger attentive applauders. Armoured aardvark assassin armies attach augmented arrows against anti-alliterater arms. Amy accounts at an abacus and all assassins advance at Aidan's adjuration. Appreciate alliteration and all authority ain't authorizing abuse against any assets. Amen.

The building was on fire, and it was not my fault. Burn down, blow up, or heavily damage 15 different buildings across fifteen different adventures using only fire or countermeasures put in place to stop you while still managing to utterly annihilate the bag guy's plan.

Well-aged cheddar: Come up with an unintended use for something both completely unrelated and way more entertaining than the intended use.
So obvious nobody sees it coming: Create and play a character with 6 or more skill points per level that goes against their normal class flavour.
Casting fist: As above, but with a character capable of arcane casting
Holy land of Rahadoum: As above, but with divine magic
Never heard of subtlety: As above, but with psychic magic
What weapon?: As above, but with full-BAB classes
Why even take those levels?: Accomplish the anteceding 5 achievements with a single character.
There's a god for that?: Surprise the rest of the party with your choice of faith.
Sequence break: Complete the campaign without using the mcguffin.
