My first experience with the Dungeons and Dragons system was with the Mongoose Publishing version of Conan that was cancelled a few years ago. In that system it was possible to make a Pirate variant whose Sneak Attack damage was changed to Precision Damage, and only applied on successful attacks of opportunity.
Then there was a feat (I believe) high up the tree that made it so any attack against you that failed provoked an Attack of Opportunity. Combined with Combat Reflexes and a high Dexterity score, I had developed a character who could effectively blender anyone who attacked him.
Now I'm looking to create an NPC who works about the same way. So far my search has been fruitless. Am I missing anything, or is this character build unavailable in Pathfinder?

I know of two ways to build for this, though neither works exactly like the one you mentioned. There might be more I'm not aware of.
The first is the Duelist prestige class. At the 5th level of the class, it gains an ability called riposte which lets you attack once when you successfully use the parry ability gained on the 2nd level of the class. I think the earliest you can get that is level 11, due to the class requirements, unless I miscounted somewhere.
The second is the Crane Riposte feat. It's part of the crane style feat chain, so there's a lot of feat requirements. It works similarly to the duelists riposte, but the crane style parry is an auto-succeed. It can be taken by level 8 if you have enough feats.
Both abilities are a bit more complicated than my brief summary, so you should read up on them before committing to building your character towards them. They also require a good deal of investment in feats.

Barbarian Come and Get Me rage power gives you an AoO for every attack attempted, whether it hits or not, the AoO is even resolved before the enemy attack. The bad side is that the enemy gets a +4 bonus to hit you, so you would need a lot of AC to actually dodge the attacks, which is why most barbarians eat the attacks with their DR and HP. But since it resolves before your opponent, you can drop him before he finishes attacking. Killing people in their own turn is always fun.
Combine with the Crane Style tree so you can negate the first attack each round and give a second AoO on that attack. Also fighting defensively offsets the penalty from CAGM, that helps too.
Go Urban Barbarian for a raging bonus on Dex to increase AC and number of AoOs and attack if you use weapon finesse. If your DM okays the Agile weapon property you can dump strenght, but know that the build will lack damage until you get that magic weapon. Another good power for that build is guarded stance that adds a dodge bonus agaisnt melee.
Its pretty feat heavy, you might want to take a level or two of Martial Artist Monk to get the prereqs for Crane Style, plus wis to AC.

Sounds like you're talking about the "Web of Death" feat, which can be summed up as "When taking a total defense action, you can make an attack of opportunity on anyone who attacks you." Prereqs were Bab+5, Combat Reflexes, Weapon Focus.
Why not just propose it as a feat to adopt to your GM?
