Switching out feats?

Was wondering when you gain a level with a feat if you can switch out any of your old ones...maybe it wasn't exactly right for your build or, you dont use it AT ALL or so on...Or is it once you have locked it in it is permanent?

BY default it is once you chose a feat it is permanent

fighters have the option of changing bonus feats out at set points
Also I recall hearing that ultimate campaign has rules for retraining feats in it

if a feat you picked does not seam to be working the way you thought it would you also have to option of talking to the DM nicely

Supposedly there were/are plans about it in ultimate campaign. Most my GMs allow rebuilds when asked.

I suggest cookies as a bribe myself, though long talks I'm prone to use cheesecake.
