As Victor slams his hand on the table the following scene appears on the table.
Lady Illirigarde appears in the table’s reflective surface, though she appears tired. A box carved with skulls is closed behind her as she furiously tries to align two delicate silver disks etched with compass directions. She looks down at a book, and lightning flashes brightly outside. When it subsides, the box is open, and the tiny winged creature is hanging from the nearby bookcase. It leaps at her throat with hate in its eyes, and the vision fades.
Yet Victor has a case of deja vu as the creature from the scene appears and leaps at him!
Meanwhile three little creatures appear from seemingly nowhere to menace the group!
Victor may take an AoO if he has a weapon in hand (or worn). If Cannoli or Vreekka have a reach weapon wielded they may also take an AoO. Knowledge (Arcana), (Religion), and (Nature) checks please.

The splash damage: I'd like the other GMs here to weigh in. I'm going to post a link to an imgur showing the grid designation for missing a splash attack. I copied Vreekka's token and added the 8 lines around it. The one with the arrow point is #1 - which, by rule, goes "off-target in a straight line toward the thrower" and then the other 7 lines are 2 through 8 in a clockwise direction. I placed a shape (circle with an X) at where I believe 8 is located. Once the other GMs have weighed in I'll resolve the splash attack.

Your image is backward because Vreeka isn’t the center of the attack, her foe is. I would say Vreeka’s interpretation of the splash on the map is correct.
Cannoli moves over and smacks the red enemy thingy, it seems that is straightforward enough for her.

She seems to pause, thinking about something, then quickly brushes it out of her mind.

Going with Cannoli's suggestion I changed my map. I knew what I intended but I didn't communicate it well which is my fault. So I changed mine to place Vreekka in her 'firing position' and made that line red. The red line is also line 1, by rule, because that's going straight back towards the thrower. Now if you look at my map, your map, and the Imgur map they all agree on one principle: Line 1 is straight back to the thrower while Line 5 is on the opposite side away from the target. So counting clockwise starting with the red line being 1, the blue line (opposite side of the target) is 5, making line 8 the green line.
#2: Let me think some more. It's irrelevant in the sense that the mites are horribly overmatched by this group so whether you kill them in round 1 or 2 is just bookkeeping. I just want to get oriented to this to make sure I know what I'm doing in the future.

Looks correct to me. My approach was that the "firing line" is more north-to-south than west-to-east. Therefore, I assumed line 1 is going straight north from the target. (Also, even if you ruled that line 1 was going northwest, the splash damage would still hit both mites. ^_^)

Vreekka is at about 11 o'clock and that's the line that becomes line 1. That line extends to the opposite side of the target and becomes line 5 (about 4 o'clock). That's going clockwise which makes the green line (roughly 9 o'clock) line 8. Again, let me make it perfectly clear: These mites stand zero chance of doing any real damage and you guys are going to mop the floor with them whether the splash hits or not. This is me just trying to learn if I have the principle correct for future situations where it might matter.

To not have things bog down I'm going to sidestep the mite 'situation' for now by adjusting Vreekka's action to target the intersection next to Green instead of the mite itself. This is only AC 5 to hit and it would eat the splash damage of 6. Since it failed it's save it's a goner. Since Green was the original target of Vreekka's throw it's dead either way.
The Blue mite, seeing the power of its enemies, seeks to instill DOOM! on a victim!
Cannoli 1, Victor 2, Vreekka 3: 1d3 ⇒ 1

She turns back to her foe and comments, ”It’s easy to say ‘keep one alive’ when you aren’t the one it is trying to stab. Besides, who are we to deny them the right to glorious death in combat?”
