I've been running a 2e game for a few months now, and I'm just wondering if I'm running Death and Dying correctly. I'm reading the rules that as long as a PC doesn't die of massive damage, they really just need to make a flat save and they are at zero. It really doesn't matter if you were at -2 or -20 as long as you didn't hit the massive damage threshold. Am I missing something?

Our game experience so far has been that it is really hard to permanently die, With most of the PCs invested in Medicine and a full time healing cleric, the PC survivability is greatly expanded, at least that's our observation so far, which we're actually enjoying. We really don't miss having to pause the game so much to take care of healing.

It's true that the amount of damage doesn't matter. PF2 doesn't use negative hit points.

When you drop to 0 HP you become Dying 1 unless the damage was from a critical, in which case you're at Dying 2. If you get hit while you're down, your Dying value increases by 1, or by 2 if it was a critical.

Recovery checks are flat checks, and the DC is based on how badly you're Dying. Even at Dying 1, you have a 50% chance to get worse.

If you lose Dying as a result of a recovery check, you're still unconscious. And your Wounded condition worsens, which also happens if you're healed. You can't recover from being Wounded during combat, so going down more than once is a bad idea. If you're Wounded and you gain Dying again, you add your Wounded value to your Dying value.

If your Dying value hits 4, you die.


Are you using the Wounded condition? If a PC goes down more than once in a fight they get dangerously closer to death... But also with 10min out if fight it can go away especially with so many trained in medicine


There is no longer negative hit points in 2e.
As of survivability.
If character will get wounded 3 and get to 0 hp - he will die instantly.
(unless he extends his dying condition to 5 through feat)
