How does the Simple Corruption Lycanthropic Creature Template work?

What exactly does it gain from the base animal? Does it still use the base animal’s ability scores in hybrid form? What about natural armor bonus?

Normally I wouldn’t question it, but it says you can change it by changing what the base animal is, but then fails to explain the full change mechanically, and I can’t find any stat block examples of this template.

It isn't called out. So, I would suggest using the beast shape line of spells to determine exactly what's gained and what isn't. By comparing what a wolf gets and what the spell grants. For example, since the template doesn't give you trip, it's reasonable to assume you get everything beast shape 1 gives you.
