Check the file name. It's probably the cover for the module The Godsmouth Heresy
The Inner Sea Primer cover definitely shows Thistletop, complete with giant head island.

I'm no yoda8myhead level continuity expert, but I think the head upon which Thistletop was built was mostly upright, leaning at a slight angle, not on its side.
That said, the picture freaking rocks.
Actually all it says in the adventure is that when the sun hits it right you can make out the face, no actual indication of it's orientation at least in the original text.

Page 39 of Burnt Offerings has a picture that has the head's orientation as almost upright. That's probably what Sebastian is remembering.

Hmm, is the right hand picture from some sort of 'return to Thistletop' module, at some future point impending? I assume it's not original Runelords artwork (or even never before seen Runelords artwork) since Seoni was the arcane caster iconic in the original Rise of the Runelords arc, not Josh Frost - err, Ezren... :)
