Going to assume Andakos put cultist #4 down. If it somehow didn't, I'll retcon this.
Golloriel makes her way through all the dead and dying to get into a position to protect her father. She slashes out at cultist #2.

Somehow, skeleton #3 survives the solid hit from Andakos.
Golloriel misses her attack against cultist #2 and Sarenrae punishes her by only healing her for 1 damage.
Cultist #3 casts an infamous spell as a spectral scythe appears next to Andakos and lashes out. It's about as useful as your GM. (Big whiff.)
Yeah it's a spiritual weapon.

It's fine, it's not like they would have come into play yet anyway. Only one enemy attack since your last turn and it was a big ol' whiff regardless.
Cal's frustration at being surrounded only increases as the constant battering of his enemies causes him to mispronounce a syllable, sending the gathered arcane energies in his hand dissipating back into the Aether from which it came.
Skeleton #3, who is shockingly still standing, slashes twice at Andakos and doesn't even come close to critically hitting, which is the only way this loser is going to penetrate the investigator's nigh-impregnable defenses.
Queen's Physician #1 strides into the room and behind Golloriel, but they pause and look expectantly at Cultist #2 (readied attack). Cultist #2 nods, steps down into a flank with the Physician, and the Physician attacks Golloriel once they have her attention split. However, even with her attention split, the Physician's dagger can't find a chink in her armor and glances harmlessly off one of her plates.
Likewise, Cultist #2 brings his scythe down on Lori's head, but she's not even there anymore, easily sidestepping the telegraphed strike. She didn't even have to bring up her shield, for gods' sakes. Who trained these people??
Physician #3, apparently sick of waiting for the skeleton in front of them to just DIE ALREADY, slides over and into a flank against Cal. (Provokes from Andakos, but only with the gauntlet, not the spear.) He then slips on the blood of one of the many dead enemies on the floor and his club swing goes so wide he doesn't even pop an image. (Nat 1.)
For those following at home, that was 5, 7, 5, 1 on the die for the last four attacks. FML.

Arterial blood erupts from the pair of wounds, and Rey spin-kicks the physician so hard their unsupported neck snaps back, cracking the exposed spinal column so their head is hanging on only by a handful of sinew and stretched skin. #3 collapses. Even as she comes down from the spin-kick, Rey slashes #2 across the chest. They don't fall, but they have a sucking chest wound now and look on the verge of death.

I believe you meant for Lori to attack Physician #1, since Physician #2 is not in range and therefore you would not get a full attack action on them.
Golloriel unleashes her scimitar on the nearby physician, but despite two devastating slashes across their torso, they remain standing--though barely. (Not quite disabled but they are clearly on their last legs.)
Cultist #3 waves a hand and his spectral scythe tears into yet another Cal, who vanishes in another puff of smoke.
That mirror image is saving your ASS.
Clearly irritated, he waves his hands and mutters a profane prayer, and subtle energies ripple around his hands, which then grip his scythe once more.
He cast divine favor, giving a small bonus to his attack and damage rolls.

Oh man, divine favor was Vislani's favorite spell. She had the Fate's Favored trait, which grants an additional 1 point to any luck bonus. At level 12, divine favor gave her +4 to hit and damage. She had a 40' movement speed and dimensional steps and there was nowhere on the battlefield she couldn't be in the blink of an eye. She was a fun character. A quickened metamagic rod would have made her even better so she didn't have to waste the first round of combat casting DF! But then you have the question of how she's wielding the rod and a 2-handed spear...

For all his magical defenses, Cultist 1 cannot withstand the onslaught of Andakos's spear. He doesn't go down, but he's bleeding profusely from multiple wounds and looks like he is seriously reassessing his life choices. Pharasma will surely take that into consideration when you send him to Her shortly.
In response, Cultist 1 steps out of range of the spear, casts a spell that closes several of the wounds he just received--though not nearly all--and spends a modicum of will shifting the scythe's focus to the wizard. However, Andakos's assault must have truly rattled him, because his spectral scythe misses the mage so hard it doesn't even pop an image.
He cast cure moderate wounds on himself.
Cultist 5 strides out of the southern doorway #6 just opened, and jabs a finger in Andakos's direction, muttering an incantation. Andakos feels his limbs seizing up...

He steps into the Horrible, Horrible, Terrible Room of Corpses, gives #3 a good study, and goes for the throat. (As always, popping studied strike on a hit. Andakos gauges his target, steps up, and absolutely mutilates him, putting his spear right into #3's throat and bursting out the back of his neck, partially severing his spine in the process. The cultist collapses to the floor like a ragdoll, unable to control his limbs anymore. He's technically not dead yet, but he is not long for this world.
Yeah that 18 was a hit too; he didn't have shield of faith up. So you did 30 damage in one round to this guy. :O
Cultist 1 faces off against Lori's imposing defenses as he stumbles over the spreading pile of his former coworkers. He attempts to draw forth some sort of esoteric power, but Golloriel's aggressive shield-work causes him to fumble the incantation. Instead, he mentally redirects his spectral scythe to attack the helpless Reyhan, hoping to force the party to close ranks once more. But he's so flustered that he can't properly direct the spiritual weapon and it gouges a hole in the floor next to Rey instead.
What a time for a nat 1. Lucky SOB's
Meanwhile, #5 decides he definitely does not want anywhere near that longspear. He steps back and conjures forth his own spiritual weapon to harangue Reyhan, who looks about ready to crap her trousers at this point. Yes, you can absolutely walk through the space unhindered. Spiritual weapons can't flank or make attacks of opportunity; to me, that says they don't pose any real hindrance in their square, they just exist until given a direct mental command to attack from the caster.

Oh, one thing did happen between Golloriel and Andakos:
The spectral scythe controlled by cultist #3 bamf's out of its spot and reappears hovering next to #3's corpse. It does not menace anyone approaching or in fact do anything except float there.
Turns out if the spiritual weapon goes "out of your sight" it pops into a a square adjacent to you at the beginning of your next turn. And since ya can't see while unconscious, that's what it did. So, while technically the spell doesn't end when you go unconscious, it may as well.
Spectral scythe #3 actually just vanishes from existence because the spell ended. Scythe #1, however, being slightly fresher, floats over to its owner's corpse and hangs out there. A huge phantasmal scythe hovering over a bisected corpse is... pretty f+&~ing metal, actually.
Cultist #6 gets rickety-*BELCH*-wrreeecked by Ducky's longspear to the chest. He's still standing, but barely.
Cultist #5 calls out, "Get out here, necromancer! They're slaughtering us!"
He then slashes at Golloriel, while his spiritual weapon slashes at the helpless swashbuckler. The paladin bats away her attacker's scythe easily, but Rey isn't so lucky. She is lucky, however, that the spectral weapon catches on her armor slightly and so doesn't dig itself into her as painfully as it could have. (Minimum damage; 2 force damage to Reyhan.)
I have once again cleared off the dead and dying. I also cleared off Thulae since he doesn't really do anything when he's not busting out the hot looks.

NGL it's been a hot minute since I saw a mirror image spell pull so damn much weight.
Furious, #6 steps over some bodies (including his friend that Andakos just skewered) and reaches out to touch the investigator but the spell fizzles because these guys suck at life.
From the south room, a figure lumbers into the doorway. It appears to perhaps have been a human male originally, but is now just a pile of fetid meat that walks. The jaw is distended, clearly broken and looking ready to slough off with a slight breeze. The necrotized flesh pulses and bloats in horrid misshapes, fluid-filled sacs and boils covering exposed flesh. The glassy eyes are devoid of any emotion as it surveys the battlefield.
This is not your average zombie. This is a plague zombie, notable from their standard cousins by the fact that they detonate on death and spread zombie rot to those unfortunate enough to be in the blast zone.

From behind the zombie, a figure can be seen peering around its bulk. Despite the impressive height of the zombie, the man is actually taller, though much, much lankier. Greasy brown hair dangles in unkempt mats that are pulled back into a filthy ponytail. Pallid skin hangs off his drooping face, and his sunken eyes bear an uncaring malice.
Andakos is immediately struck by how familiar the man looks. He's quite sure he's never met him before, but the notion that Andakos knows this man is almost unbearable--until it hits him.
Shrink him about six inches, and put a good forty years on him, and Rolth Lamm would be the spitting image of his father, Gaedren.
"Ah, you must be that tiefling who attacked my father." He waves a dismissive hand before Andakos can respond. "No no, don't worry, I'm not upset about it. You drove the decrepit old wretch right to me, and I was happy to have some extra parts for my experiments. No," he adds, his face twisting in anger, "what irks me is that you slaughtered Vreeg, a rather apt pupil. Certainly better than that daft bird, Jolistina." He sighs dramatically. "Ah, well. At least you will make for some fine parts."

The spell he attempted was ghoul touch. Had Andakos failed his save, he would have been paralyzed for the duration. In addition, he'd have exuded a hideous stench in a 10-foot emanation; all living creatures in that radius would have needed to make Fortitude saves or become sickened.
"Well, that's vexing," Rolth grumbles. He calls behind him, "Get in here, you useless meatsacks, and kill these trespassers!"
Another zombie shuffles up next to him in the doorway, and more sound can be heard deeper in the room.

Lori's first scimitar swipe misses, but the second digs a deep furrow in the cultist's side, in between two layers of chainmail. The cut doesn't seem as deep as it could have been, but the combination of it and his previous wounds result in a loss of too much blood, and he wavers in place before collapsing to the ground.

Earnest pops onto the material plane and quickly assesses the situation. Whoa, that's a lot of dead bodies. Let's make more!
He steps toward the zombie to the south of him (#1) and slams it.
slam: 1d20 + 8 ⇒ (1) + 8 = 9
damage: 1d6 + 11 ⇒ (5) + 11 = 16
Ugh.
---
spellcraft: 1d20 + 14 ⇒ (5) + 14 = 19
Ah, ghoul touch. I have a wand for that but everyone is always too close to each other! Stupid 1e!
spellcraft on spectral hand: 1d20 + 14 ⇒ (18) + 14 = 32
I'm assuming that's enough for him to know that he can kill it.
Cal moves past the floaty scythe and into the next room. He uses his pearl of power to recall magic missile and fires off two missiles at the spectral hand and the third at zombie #1 (which he can now barely see).
MM @ hand: 2d4 + 2 ⇒ (2, 3) + 2 = 7
MM @ zombie 1: 1d4 + 1 ⇒ (1) + 1 = 2
