It's one reflex at the end of your turn. The first reflex was to cover the time from when the spell was cast to the end of your next turn. If you start the turn un-entangled, you can act that way, and then see at the end of your turn if you get entangled at the end of your turn. You are clear of the entangling growth but still in the difficult bog terrain.

Okay, so, if you'll let me use that 17 I rolled at the start of my last turn to count as the one at the end of that turn, then I'm not entangled and I don't need to make two concentration checks to cast. But if not I'll roll a second one.
However, before I do that, question: does it look like the croc is merely defending the kobold, or does it want to make a snack out of my dwarven friend?

Tiann: Make a difficult sense motive check if you want to understand the motivations of the crocodile. You can also make a knowledge(nature) to get a bonus on the sense motive check. I'd rather not rewrite too much history retconning rolls so we'll ignore the extra roll last time and stick with you getting entangled at the end of your last turn.
Jory: You are correct, I miscalculated your penalty, you broke free, missed the croc, and avoided getting re-entangled. You are free for your next turn.
The crocodile sticks to his current favorite meal, the intrepid Karzak.

Jory, give me a climb to get over the log. At a DC of 15, you can get on top of the log with 20 movement, at a DC of 20 you can get on top of the log for only 10 worth of movement.
Keep in mind that the 3 spaces NW, N, and NE of Jake are all difficult terrain and require double movement. If all you are doing this turn is moving, your total movement quota is double your normal speed, for a double move. So you have a max of 40 movement total available. If you fail the climb by 5 or more you fall prone on the ground.
That acro will happen at the end of your turn, so you'll be entangled again at the end of your turn if you fail to climb onto the log or don't have enough movement to do it.

Not trying to be a really harsh GM, but I'm not big on changing your action once you've failed at something...the attempt to burst the vegetation has already happened, you still get any standard action that you want (including using your standard action as a move action).

@GM Fnord: If you say so...the goal was to attack the frog, and had I known that an attempt to disentangle myself was not part of my move action, then I would have assured myself the ability to do so, whatever the mechanic. Seeing the croc fall and Karzak back in the fight, Tiann focuses her attention on the frog that appears next to her. She stays quiet during the fight, focusing her attention on keeping people alive and downing enemies—though she does grin a bit at Tal's repartee. She makes a slashing swing with her blade, and maybe seems a bit eager to exercise her sword craft.

After this she makes an attempt at freeing her feet from these thrice-damned vines.

The angry Kobold swings her club at Tal.

The frog turns to attack Tal as well.

Jory - there's a 5' tall log in that direction. There's nowhere you can move out of the entanglement other than climbing the log (which will require breaking the entanglement first or suffering a big penalty on your climb check). Decide if you'd like to try to break free and then climb, break free and then move in some direction other than up the log, or not break free and climb / move / do something else.
Tiann and Tal both swing and miss.

Well, as I don't want to spend a possible fifth round completely useless, and standing uses my move action. It is of no value to spend the rest of my action to break free and stand in the entangle to start the next round.
Jory stands and does his best to move free of the entangle struggling to get the last little bit out of the spell.

Tal looks down at the two pieces of Kobold; utterly impressed with the killing blow that Karzak delivered. He sees the undergrowth retreat back into the ground, freeing his friends from the tangle.
Tal searches the Kobold's person (what's left of it anyway) and around at the Kobold's "home"...taking care not to disturb any other hidden crocodiles.
