Okay, I think the added labels might have confused more than helped. You guys have yet to go on round 1, and the shark's next turn will take place at the start of round 2. I just put the round 2 label on so the separation between the end of one round and the start of the next was visible.

Vyshael, since you stated you had readied that action before combat and init, you interrupted the shark's attack. Meaning you are now first in init order and your spell took effect in round 1, so I used the attack and damage you posted for round 2.
Vyshael had expected the shark's reaction, and as he had planned was able to dart forward with an ensorcelled hand full of electricity to zap the fish just as it got to Raliscar. Sadly, it wasn't quite enough to kill the thing.
Sorry Neva, the map is 10' squares, so your penalty is -10 for range, with the additional -4 for cover.
Neva had a bolt loaded and ready to shoot, and did just that, but sadly the thing got stopped by the water a good ten feet away from the thing.
Both Felgar and Rutter do what they can to close the gap between themselves and the fight, but it takes all their time just getting there.
Raliscar takes a chance, and decides to use his crossbow in close quarters, regardless of the risks. It was worth it, as the shark just can't seem to get a bite on the dwarf, and instead gets a big heavy bolt to the jaw for its efforts.

Seeing the shark beginning to retreat, and wanting to ensure it doesn't return, Vyshael points his blade at it while tracing a pattern with his webbed hand, causing three darts of force to streak out and strike the beast.

Vyshael's three glowing darts of force make the shark's attempt at escape fail quite certainly. It turned to dash away, but got nowhere before stopping to float belly up.

Raliscar pumps his arm jubilantly as the shark starts to turn tail but his gesture turns into a whoop of victory as Vyshael brings the predator down.
"Dumb thing should'a jus' calmed down like I told it to. Stupid fish." He glances to the hole the shark emerged from, then casts a wary eye toward the rest of the wreckage and sees a good many places for other sharks to hide. Shaking his head, his beard trailing through the water with the motion, he pauses to reload his crossbow before continuing on toward the boat.
"Do sharks like to work in packs, like wolves, or are they loners? Anyone know?"

Vyshael says, "Think it depends on the species.. Might be worth seeing if our friend ate anything useful... But the blood might attract more of them."

Rutter |

Rutter puts his blade away and continues to the unexplored part of the wreck. "Cut him up if you like; I'm no good in the open like this. I'll keep searching."

Vyshael says, "We should all keep searching -- just tuck the shark into a bulkhead so it doesn't float away and we can do the cutting him open when we're back above the surface."

As Vyshael blasts the escaping shark, Felgar swims closer and Raliscar reloads his crossbow. Rutter and Neva, however, attempt to make forward progress, but only flounder in the water.
From out of the hole where the shark emerged, everyone but Rutter and Neva can see a hideous being emerge, their limited eyesight hindering them. It is a woman, in the loosest sense of the word. The thing is so hideously ugly, the sight of her almost drains you to your very core. As soon as she emerges, she begins casting a spell.
Felgar, roll a DC 15 Fort save, or take 1d6 ⇒ 2 Str damage.
She falls under Knowledge Nature for monster lore.

Neva, remember, each square on the map is 10', and a single swim check can get you 15' (by beating the DC by 5). Even then you wouldn't make it to Felgar.
Raliscar, sadly, no matter where I move you I can't get you closer than 35'. That nets you an additional -4, which does not hit.
As Raliscar moves forward and gets a better look at what swam out of the hole where the shark originally emerged, the hideousness of it strikes him with a feeling of queasiness. This sparks a memory of something you had learned. This thing is clearly a hag, specifically a sea hag. You know an amazing amount about these things. Unfortunately, his bolt didn't have enough force after going through the water to pierce her tough hide.
Raliscar, roll a DC 15 Fort save, or take 2 Str damage. You beat the knowledge DC by enough that you can just use the whole monster entry.

sorry about that. I'll then continue in round 4 with a second move action to swim. In round 5, I'll swim as a full-round action. The chart says a single move give .25 speed, or .5 speed on a full move. I'll assume that I took two single move Swim checks in Round 4 and then a single full-round check in Round 5

As most of the group do what they can to swim toward the hag or disrupt her spell, they are successful at the former but not so much the latter. Due to this, she gets the spell off, and in the space right beside her and Felgar appears another shark. This she promptly commands, in some unknown language while pointing at Felgar, to most likely attack him. That is exactly what it tries to do, and manages to gnaw on the sturdy dwarf's shoulder, and a cloud of blood floats out into the water.

The dagger shimmered and wavered in the swirling water and failed to find its mark.

Casters, Vyshael discovered that the Hag has at least some degree of Spell Resistance. If your spell is affected by such, please go ahead and roll your caster level checks to overcome it.

Yes, it is 1/4 or half, but I included a house rule for climbing and swimming, where you can move 1/2 instead of 1/4 if you beat the DC by 5 or more. I just hate how no matter how good a swimmer you are, you can never move faster than 1/4 your base.
Vyshael grows a wicked set of horns, and darts through the water, twisting agiley underneath the shark to stab the hag with his new weapon.

Neva swam closer, and catching sight of the woman cast a spell materializing into a dagger of pure force. As the weapon zipped effortlessly toward the hag and swung, it just dissipated in to little motes before vanishing completely.
Felgar stayed focused on the creature chewing on his shoulder. Swinging with all his might at the big thing, he was hindered by the torn muscles and ligaments in his bitten shoulder, more blood clouding the water as he moved the limb.
Rutter made headway toward the hag, but now both he and Neva were both close enough to see the gut-wrenching hideousness of the woman, as it weighed heavy on them.

Raliscar thrashed about in the water poorly, before stopping and just reloading his weapon.
Drawn by the blood coming from the dwarf, the shark continues to bite at Felgar, but doesn't get as good a chunk as before with the clouded waters.

The hag, chooses this time to square off against the somewhat narwhalian elf that had just gouged her. She jabs twice at him with her shortspear, and slaps with a scaly, webbed, claw of a hand. For as gangly and ugly as she is, the hag has an almost supernatural power behind her attacks, and thrashes Vyshael thoroughly.

Vyshael winces at the blows, part of his mind noting how the water didn't slow the hag's claws and realizing that he's outmatched, while another part regrets not having released the full extent of his rage to drive away petty concerns like pain and doubt. He closes both hands around his mother's sword and tries to drive it through the hag with every ounce of his enhanced strength... Hoping to hit something vital before she impales him, especially now that she torn away his buffer of false life.

Trusting in the stalwartness common in all Stoutaxes, Raliscar turns his aim toward the greater threat of the hag. Praying to both Erastil and Gozreh under his breath, he looses another bolt.
