So if an enemy rolls a success they start at stage 1 and cannot advance to stage 2, I get that. If I was to not sustain the spell then the entire spell drops off. If I do sustain it, what happens? Does the enemy get a saving throw each round, half damage on a success? Then say third round they fail the save they just take full damage instead while still staying at stage 1? Basically they wouldn't be able to remove the curse as long as I am sustaining for up to 1 minute?

When you sustain the spell, the target then needs to make a new Fortitude save (against your Spell DC) or take the void damage and become fatigued.
If they pass their Fortitude save, then nothing happens that round, though they remain cursed for as long as you continue to sustain the spell. If they fail their save, the curse remains at Stage 1 and they take the damage and the fatigue.
At least that's how I interpret the spell.

I don't know. If we think that success on a save can't decrease curse's stage below 1 - then they still are at stage 1 and must receive damage. So it's unavoidable until you stop sustaining. There's no affliction state 'virtual stage 0'.

Nothing in the spell description says you can't decrease it (it would need to say specifically that "this curse cannot be reduced below stage 1").
What happens is:
Enemy makes the save, immediately goes to Stage 1 and takes the damage+fatigue.
End of the enemy round (or start of your next round, I've seen afflictions ruled both ways), enemy makes the save,
a)if he makes it, the curse is over, like every affliction when you save vs it when it's at stage 1.
b)if he fails, he can't increase the stage (since the new maximum stage is 1), so he stays on stage 1 and repeats the Stage 1 effects (the maximum stage for the curse for him), like you would for any affliction when you are at its maximum stage and fail the save.

lists an effect followed by an interval in parentheses.
When you reach a given stage of an affliction, you are
subjected to the effects listed for that stage.
At the end of a stage’s listed interval, you must
attempt a new saving throw. On a success, you reduce
the stage by 1; on a critical success, you reduce the stage

There ARE specific curses/afflictions that don't allow you to drop it off by simply making the save, but all of those explicitly state so.
As an example, mummy rot's
This affliction can't be reduced below stage 1, nor can the damage from it be healed, until it's successfully treated with remove curse or a similar effect;
Curse of death doesn't have that ability.

Hello, I am answering from what I understood using the rules in player core.
First of all, Curse of Death duration is "Sustained up to 1 minute"
The entry for Curse of Death states "This curse ends when the spell ends"
So basically sustaining the spell, prevents the curse from falling off the target. Now to the second part.
A Curse is an Affliction, and it has a initial saving throw, and subsequent saving throws. The success on the initial saving throw prevents it from going into further stages, but as it is an affliction, every time the time in parenthesis goes by, a subsequent saving throw has to be made. For this case it is 1 round.
In this spot the 4 degrees of success will have the following effects:
Critical Success: Reduce affliction by 2 stages
Success: Reduce affliction by 1 stage
Failure: Increase affliction by 1 stage
Critical Failure: Increase affliction by 2 stages
So depending on the result you move the affliction the number of stages and apply the described effect for it's current stage. In this particular case, Success and Critical Success should move the affliction to stage 0, and hence dropping the affliction (no need to sustain anymore) or they can fail, and the stage will not increase, since they succeeded the initial saving throw, and can't go past stage 1, they enter stage 1, and get the stage 1 damage applied again.
So why do you have to sustain the Curse of Death, if you don't sustain the spell, the curse drops.
What remains uncertain to me, is if the fatigued falls off if the curse falls of. Thinking about it as part of the affliction, it should, but looking at the Fatigue effect, it should linger until they take a full rest.

Correct, Fatigue lasts until you get rest or remove it by other means.
Conditions affect you when you reach the stage and last for their normal duration. For instance, if you were drained for an affliction with a maximum duration of 5 minutes, you remain drained after the affliction ends, as normal for the drained condition. A condition that automatically changes its value or ends under certain circumstances, like frightened, still does so. Any condition that doesn’t have a default duration, such as clumsy or paralyzed, lasts as long as you’re at that stage unless noted otherwise, as do any penalties or any other effect of the stage that doesn’t list a duration.
