What is the effect in game terms to wielding a sword that constantly belches out smoke?
Is it like the smoke from a torch or is it smoke that obscures vision and causes problems?
Since there is no specific game info that I can find anywhere, I'm thinking of just making it so that when it is drawn, it is whispy smoke like from the embers of a flame. Nothing more.

It doesn't cause any problems beyond what its stats give you

As you can see from that entry, the smoke is 'flavor' text, not rules text. The 'rules' text includes the following:
It has all the attributes of a longsword.
It has all the attributes of a +1 magic weapon
Any hits do an extra +1 fire damage
There is a special action available while you're weilding the sword. The special action uses 1 action and has the 'concentrate' trait.
Effect: until the end of your turn, the blade does an extra 1d6 damage, instead of just +1.
After you use this action, you can't use it again for 10 minutes.
So the smoke doesn't do anything else except give your story visual details. It does not have the effect of obscuring vision. It does not cause any problems.
