Same Disease Different Save DC

Rules Questions

I am a little confused as to where exactly the save DC is coming from, or rather, if there is a mechanic I am missing somewhere that makes a save different than the one in the Core Rulebook. Examples below to illustrate my question.

FILTH FEVER
Type disease, injury; Save Fortitude DC 12
Onset 1d3 days; Frequency 1/day
Effect 1d3 Dex damage and 1d3 Con damage; Cure 2 consecutive saves

Wererat

Dire Rat/Rat Swarm

Now I understand why a DC might be higher or lower from creature to creature (bigger bite, more bites, etc) but I was hoping for some clarification as to how the new number is generated.

Any help is appreciated.

The DC formula for effects from creatures is 10+ 1/2HD+ relevant stat mod. The stat mod is usually listed in the creature description.

In this case both the wererat and the other named creatures use their constitution modifier.
