Last Will and Testament

At one point I saw a Fantasy Character Last Will and Testament but it was a long time ago.

Does anyone know where I might find one?

The 1st Edition AD&D "by class" character sheets (eg Fighter/Ranger/Paladin, Cleric/Druid, Magic User/Illusionist, etc) used to have a small will at the bottom of the back page.
