Never let combat bog down. If there's a rules question, ask a player to look it up and move on. If someone is taking an inordinate amount of time, tell them that they have a few seconds and then are being put on delay and can come off delay when their character is ready to act.

I always keep index cards with me - I use them for initiative, secret messages, active spells, you name it!
