In an ambush situation, you're free to decide your order of action, if you like. It only becomes important if some on the opposing side aren't surprised, or if two or more of you really want to race each other, like, for the last lifeboat, or something.

Just so you know, I allow 'partial charge' in the surprise round, though it's legality is hotly disputed elsewhere on these boards.
You easily clear the distance between you, and swing your hammer, catching the goblin in its downward path, and spinning it into the corner to lie in a twisted heap.

It's very cramped in there; Ensu doesn't want to waste a spell before he knows what the opponent is, so readies his longspear to guard the square immediately outside the doorway.

Before Valoria acts, there is a loud bellow from the room.
Kavrik appears to be being battered by a large flail from round the corner. The part of the arm you can see is long, brown and hairy.

It appears to be a mighty blow indeed.
Kavrik's look of disgust is wiped from his face in one punishing blow, that wraps over the top of his shield, to knock his helmet over his eyes. He drops with a resounding clang.

Whoops, forgot to say, the attacker steps over Kavrik, after the attack. You are all able to see he is a large brown, furred goblin, with a physique like Galstok, and a double-handed flail in his hands.
And he is wearing no pants, and was obviously in the middle of something.

He roars as the missile hits.
You can all now see he is wearing most of a suit of studded leather armour, across the front of which is stretched the skin of a face. A necklace of long, shrivelled ears hangs round his neck, and other trophies are attached to his ears and hair.

Turning at the sounds behind him, Galstok catches sight of Kavrik lying crumpled on the ground. With snarl Galstok launches himself at the beast swinging wildly.

The goblins leap up and down around Galstok, hitting him with dirty plates, and jagged broken bottles. Their screaming has risen even louder.

I'm not treating these as improvised weapons; stat-wise, the goblins are supposed to have normal attack values, this is more flavourful.

Tregar's action may alter depending on whether Sephird stepped forward to fill the doorway, or stabbed round the side (and whether the blow was affected by a miss chance).
I'm going to assume he took the more direct option, unless I hear different by tomorrow morning. It also blocks the creature's exit from the room and protects the unarmoured casters.

my apologies, I thought my actions were clearly stated, since Sephird could move to flank he would do so, and attack. If he could not flank, then he would step back to continue ranged attack.
Thus he dropped the bow, stepped forward while drawing his weapons, and critically stabbed the goblin with his rapier, causing 6 damage.

Now I'm NPCing for three PCs, the rounds sometimes go faster, but it may not be clear that someone's acted, if it's my avatar describing it. Would it help if I created stand-ins for Ensu and Kavrik?

This also means you do extra damage for sneak attack: 1d6 ⇒ 2, for a total of 9 damage.
You appear to be wearing him down; these attacks are not just exhausting him, but drawing blood.

The goblins continue to swing their dinner plates, cutlery and broken furniture at Galstok, maybe sensing he is tiring. Their screaming has developed a sing-song quality, and they have reverted to pidgin Taldan.

Just before Ensu's turn, Tregar shouts out.
"Watch yourselves! We have company!"
All in the main room hear this, as does Galstok, even above the din.
Ensu became aware of the same thing, at the same time.
Ensu, the face looked human. Tregar had less of a direct view, and decided against taking such a shot.
GM note: I use the optional 3.0 style rule for varying degrees of cover, rather than always being a flat -4. I also allow someone who is readied to guard more than one entrance, as long as both are close together in a tight arc.
Ensu is next, followed by Tregar to decide to act or remain readied, then back to top. Time for a break.

Seeing the door opening next to the current battle she changes from using her final 1st level spell, keeping it instead for the new comer once she can determine if new comer is a treat. Instead she casts her daze cantrip onto the problematic goblin, hoping to slow him down enough for Galstok, and Sephird to finish off the goblin. Even hoping for Kavrik to wake up and run his sword upwards into the goblin above.
