In all cases it's bad for the revived character. Firstly they're prone and empty handed, secondly the enemy is right on top of them making for easy AoO's if the revived character does almost anything, namely reaching for a dropped weapon if they have none sheathed.
I would rule that the enemy must move out of the occupied square on it's turn which does not provoke into any legal adjacent space. If that cannot be done then both creatures take the penalties for squeezing.

Whichever character acts first, out of the revived PC and the enemy, must end his turn in a legal square.
I wouldn't allow the character to take combat actions (including delaying) while in a square he's not allowed to occupy, except to resolve that condition. If the first character to act made a genuine effort to move, including drawing AoO, but failed a check, I might then move him to the closest legal square.

I've had this exact situation last session... Twice : once with a PC wanting to go in a prone PC square (the prone PC has just 1 HP left so he wanted to protect him) and a monster in a PC square and a good channel energy reviving him...
I used the squeezing rule for both case... With the difference that in the second case the PC quickly tried to get away before the creature take his 5 foot step and kills him... :p
If neither can move because of circumstances then I stay with the squeezing rule (and the creature will try to kill the troublesome man at his feet first :D )
In case of the pit, I said If neither is dead 'cause of the falls damage then go with the squeezing rule and an awkward fight... :D

Saying that a body in a square is difficult terrain sounds good, until you have a bunch of mooks littering the floor and then it gets to be a pain to correctly count movement.
Many of my combats occur in restricted areas that might only be 20'x30', for example. That means only 24 valid squares for Medium-size creatures. If you put four party members and 6-8 mooks in the room -- it gets crowded!
I often ignore bodies as difficult terrain, but most inanimate objects will be: large desks that a creature must jump onto to cross, for instance.

What happens if a fallen PC falls, the battle somehow moves with an enemy moving over him on the same spot (since he's helpless) then another PC heals the fallen PC?
I had this happen in a game several months ago and one or times since then.
I think what I did was to give preference to the first creature to enter the square. If it became helpless (i.e. dying), then another creature could enter the square of the helpless creature, but if once the creature was no longer helpless, the "invader" had to return to its last legal square. Luckily, there was always room to do this in a way that made pretty good sense.
Although, the example of a zombie at the bottom of a 5x5 pit would not work for the way I handled things. I suppose the creature that fell would most likely be prone and maybe I would rule that it could not stand while the zombie was standing because there would not be enough room. Or if I let the fallen creature stand, squeezing rules sound good.
