So has any DM have a group not rescue Ameiko in time?
Has it impacted your campaign much at first?
If Tsuto gets away, or makes it clear that he was working for someone else, Ameiko's death could provide the PCs with pretty strong motivation to move forward with the adventure. Her role in later parts of the path can be replaced by another pigtailed NPC if need be.

My group didn't rescue her in the Glassworks, and then they were slow getting to her in Thistletop. Because of their activities though there were more goblins for Nualia to sacrifice, and I decided that Tsuto would have delivered those up before sending his sister under the knife. After all, he wanted her to join him and was shocked (and hurt, and insane) when she declined. After he had some time to consider I decided that he didn't actually want her to die and instead helped her escape when the PCs provided a distraction.
That said, it doesn't derail the path if she dies. Someone else (presumably a relative from Magnimar) inherits the Glassworks and the Dragon, and you carry on from there.

Nah, he's a criminal and after dying in the manner he did, his soul might not want to be returned from the Boneyard.
