The last sentence for the Disarm combat maneuver reads, "If you successfully disarm your opponent without using a weapon, you automatically pick up the item dropped."
I like this - it gives the option of wrenching something out of someone else's hands. But as it stands, to do this, you must not have a weapon in your own hands, which gives you a -4 penalty on your attempt.
Should there be a way to do this when you have a weapon in one hand, and your other hand is free? Perhaps if you exceed the DC by a certain amount, and you have a free hand, you can catch the dropped item in your free hand? (But still let it be automatic if you are unarmed.)

I like the idea of exceeding the DC allowing you to catch the weapon in a free hand. It creates an nice swachbuckler image.
Alternatively, the Improved Disarm feat could be changed to allow it (and maybe do away with the -4 unarmed penalty too).

Disarm doesn't address the issue of locked gauntlets, or the problem where your opponent can just pick up their weapon again. I offer the following upgraded wording:
DISARM
You can attempt to disarm your opponent as part of an attack action in place of a melee attack. If you do not have the Improved Disarm feat, or a similar ability, attempting to disarm a foe provokes an attack of opportunity from the target of your maneuver. Attempting to disarm a foe while unarmed imposes a –4 penalty on the attack. PFRPG A1
If your attack is successful, your target drops one item it is carrying of your choice. If your attack exceeds the DC of the target by 10 or more, the target drops the items it is carrying in both hands (maximum two items if the target has more than two hands). As well, if your attack exceeds the DC of the target by 10 or more, the weapon is flung to a square up to 10 feet away in a direction of your choice. If your attack fails by 10 or more, you drop the weapon that you were using to attempt the disarm. If you successfully disarm your opponent without using a weapon, you automatically pick up the item dropped.
Note: A defender wearing spiked gauntlets (page 118) can’t be disarmed. A defender using a weapon attached to a locked gauntlet (page 124) you must fail your Disarm check by 20 or more to become disarmed.
