LN Tiefling Monk from Cheliax, Gestalt Level 8 w/ Ftr

Setting
Molthune has blitzkrieged its way to conquering Nirmathas, Lastwall and Druma. Now they focus on the Orcs to the North and the Elves of Kyonin.
Concentration camps sporadically fill Molthune's giant interior, using captives of the "lesser" races and human ethnicities to do the menial work of keeping the machine of Molthune expansion moving, while the few surving Nirmathi Rangers work as guerrila Freedom Fighters against the nazi empire.
And two Chelaxians, in part of an agreement with a noble elf of Kyonin, attempted to rescue the unfortunates held in a Molthune concentration camp where some elves were being lined up for the gas chambers.
Mission Fail
The two Chelaxians faced off against the two commandants of the compund. After the Monk sundered one Molthuni's Full Plate armor, the Fighter responded in kind with a Full Attack including a critical hit, dropping the Monk to unconsciousness. Meanwhile the other Molthuni shot arrows at the flying Magus/Wizard Chelaxian Tiefling.
-- And since the Molthuni Fighter couldn't reach the flying Chelaxian and his comrade could, he executed a Coup de Grace on the unconsciuos Monk (convincing the other Chelaxian to Dimension Door away and then flee back to Cheliax in shame).

The Gory Details: The thief decided to pick a fight for some reason with some city guards and found herself hacked by pole arms and dumped into the canal.

The Gory Details: The dwarf was in the wrong place at the wrong time when a wandering red dragon decided to pick a fight. The dragon picked up the dwarf and after gaining some altitude proceeded to drop him to a painful but quick death.

Just before the final blow was struck, the villain abandoned his corporeal body and reanimated his now dead Beast, letting out one last cone of flame from his maw before they finally put him down.
Nesturuk and Ornella had high Reflex Saves
Gogo had enough HP left to survive the full damage.
Brian had neither.

Let us bow our heads in rememberance of Imperator Grakus level 13 half orc barbarian who upon descent into what can only be described as hell in a valiant effort to save the city of sandpoint from the vile Runelords.
Upon further descent he and his noble traveling companions encountered a fell creature from the bowels of the earth(Glabrezu). The Imperator charged the beast and inflicted a terrible wound, however, the beast did not fall and managed to summon another one of his brethren.
Fortune was not smiling on the imperator for the second round of combat as the beasts attacked him hitting him 10 times bringing the imperator to a bloody but glorious ending.
Can I get an AMEN!

TPK in Pathfinder Society last night.
5 Pathfinders got killed in 3-09 "The Quest for Perfection" Part 1.
The Ninja was the first to go: A hit in the surprise round Staggered her and she Withdrew (to avoid an AoO), falling to -1, 30 feet from the BBEG.
The Paladin was next, and the only PC to completely die, going 3 points below his CON score. For some reason the Cleric decided to stand toe-to-toe with the BBEG after the Pally fell. He dropped to -4 the next round.
That left the Bard and the gun-boy, both backing away and hoping to finish the BBEG off from range or flee. They were sooo close. The BBEG charged and (on a roll of 50/50 from the DM) went for the Bard, who fell to -1. The gun-boy had a choice, full Withdraw or take One Last Shot just in case -- the Pathfinders knew they were close to dropping the BBEG.
The gun-boy missed. The BBEG dropped him. -10.
So, 1 PC ALL dead, 4 PCs mostly dead.
TPK.
BBEG eats well.

"Voice in the Void"
spoilers:
The first Pathfinder to die was a 1st Level Human Fighter for the Grand Lodge -- Fell to the OwlBear Zombie.
The others died while fighting Imrizade Blakros.
The Aberration Sorceress used her Wand of Color Spray to devastating effect, knocking out 2 other 1st level Pathfinders, one a Rogue for Qadira and one a Ninja for Cheliax. Of the remaining Pathfinders, both 1st level for Osirion, the Magus fell to a second Color Spray just before being able to withdraw and the last, an Inquisitor was able to flee and survive.

Circumstances: Fell to the infernal wound of a bearded devil's glaive and bled out.
Setting: PFS #4-11 The Disappeared
Luckily, the player had the awesome Goblin Bag boon from PAX, which allows him to have the bag burned over his corpse and restore him as if raise dead had been cast with no negative levels.

Circumstances: An Ogre struck him with a mighty first blow whilst he was healing the party fighter. This left him weak enough to be killed outright by a second almighty blow.
His sacrifice allowing the fighter to slay the Ogre.

PC’s Name: Kihvet Zaumee Harr
Race: Human (Shaaran)
Class: Rogue 7
Circumstances of Death: Somewhere along the way, Kihvet started to believe she was an upfront, toe-to-toe, melee character. This misapprehension was soundly corrected by the multiple attacks a vicious gargoyle.
In Kihvet’s defence, she was quite a tough little rogue, and a slew of previous encounters featuring hordes of low HD goblins and zombies had fostered her belief she was the ultimate badass. Furthermore, she was one of the few PCs in the underequipped party with a magic weapon, so when they entered the grand tomb of the dungeon and disturbed its guardians, four gargoyles and four lemures, she went straight for a full frontal assault, longsword+1 in hand.
Claw, claw, bite, gore swiftly put her on the floor (now there’s a lovely rhyme!) and bleeding out. The gnome cleric’s channel energy revived her, but the gargoyle standing over her, with no better enemies to hand, nonchalantly put her back into negatives. She stabilized and rogue crawled out of the gargoyle’s reach, where a waiting ally passed her a cure potion.
Back on her feet, Kihvet saw the gargoyle beating on her less able gnomish companions, and despite her injuries being already grave, couldn’t resist a perfect opportunity for a sneak attack.
She missed.
The gargoyle spun around, struck her with all four of its attacks again, critting with one, and sent her soul irrevocably to the Happy Hunting Grounds.
Setting: Forgotten Realms
Adventure: The Crypt of Istaris, Dungeon#9. Kihvet’s unlikely to be the first victim of this adventure, as the dungeon blows up four hours after the PCs enter. Our party are nowhere near achieving their quest with an hour to spare. Luckily enough, all the participants are fairly burnt out on this campaign, so the imminent TPK isn’t really bothering anybody. More interesting will be to see if any other PCs fall before the clock reaches zero, the last encounter more or less exhausted their resources.

Location: The Talonquake (southern Narlmarches)
Catalyst: The King of the Forest
The gory details: The PCs finally decided to take care of the King of the Forest in the southern Narlmarches over the past three sessions. After making their way into the dragon's fungal lair and slaying his plant-based minions, they eventually retreated after defeating the King's three half-dragon Hobgoblin children and capturing one alive for questioning.Later that night, the King - Nuozelas, a Mature Adult Forest Dragon - and his consort - a Green Dragon of the same age - pursued the party and attacked in the dark of just after midnight. The party quickly took out the Green, and then started ganging up on Nuozelas, including a lucky pair of criticals: first by Lilith, which reduced the King's movement speeds, and a second by Jaekah, whose adamantine axe cut straight through his DR and left a nasty gash across the right side of his face.
Furious, the King began a barrage of attacks on Jaekah. Had he not been mythic, the King would likely have ignored him after his first attack dropped Jaekah to negatives, leaving the unconscious general to fall to the ground below and turning the rest of his attacks on the equally-nearby Takeshi. However, Jaekah now had the Mythic ability that allowed him to stay conscious in negatives - so Nuozelas kept battering at him, expending an entire full-attack of bite, claw, claw, gore, and tail slap on the general, reducing him to exactly negative Jaekah's-Constitution-times-three.
As Duchess Elegy was out of 5th-level spells for the day, she was unable to breath of life him back to life quickly; instead he'll have to wait to be raised the following morning. Thankfully the party already has a diamond on hand.

Circumstances of Death: Surprised and eaten by a lurker. Raged and enlarged to escape the grappled, was dropped by the lurker once he hit zero, and he ran out of rage ran on the next turn, killing him instantly.
Setting: Sahaguin cave, same book and AP, but a replaced fight (Congested cave water combat is annoying, and four sharks are a pain. Replaced with a lurker).

"Vampires and Drow of Varisia" Campaign
(Homebrew using "Lady of the Mists," Carrion Crown, Second Darkness, and other published adventures -- all taking place in Varisia)
After infiltrating the ruins of Xin Shalast deep in the Kodor Mountains, on their way to The Dark Tower of Cabilar near Xin Shalast (Dungeon #1) the PCs attack a group of Stone Giant Rune-Mages under the command of Mokmurian from Jorgunfist.
After the party tank fell to unconsciousness in melee with the last giant, the giant threw one of his magicalized-boulders at Morvius -- who failed to heal up after the last few rounds of initiative. (The fight progressed over a few non-combat chase sequences across quite a bit of terrain.)

After the final Stone Giant began to flee, Morthoron, already heavily injured, continued the chase. Normally this would have been no problem except that his partner in front-line fighting, Bellaris the LN Halfling Monk/ Barbarian could not keep up the chase with him, after having exhauseted his rounds of "Quiet-Storm" -- NOT Rage because he's a Monk/Bbn and not just some dumb Bbn! -- and gained the Fatigued condition. By himself cornering the Stone Giant, Morthoron had no chance. Especially with a critical hit that smashed him into oblivion.
DM Note: I allowed Morthoron to use a Badd-Ass Point (I don't give lame Hero points, PCs in my Campaigns are Badd-Asses!) to halve the damage and only be unconscious & bleeding, 2 HP from his CON score, even though Morthoron had used his Badd-Ass point already.
