I'm a big fan of the elemental templates from the Book of Templates by Silverthorne Games. Gets the flavor and appropriate mechanical powers across without changing a lot of math like lots of other templates do.

I've never used it but I love the Titanic template from MM2. A freaking toad big enough to crush a house is just funny.

Paragon (ELH)
Phrenic (EPH)
Monster of Legend (MM2)
Lycanthrope (MM; plus variants all over)
Spirit Creature (Frostburn)
Shade (various FR supplements)
Evolved Undead (so cool!), Spell-stitched (also!)
Tauric (MM2)
Titanic (MM2)
Spellwarped (MM3? MM4? One of those.)
And Half-anything (because they're really cool!)

But my absolute favorite has to be not-actually-a-template-but-called-one: Living Spell.

So. Cool.

EDIT: Also, because I forgot...

I have returned form the dead to take over the worl- oh, holy craaaaaap! *its head rolls off the side of the screen*
