I once, in the last millennium, played (2nd ed DnD) with a guy who insisted all characters started at 1st level. It didn't matter that most of the group was 7th+ and dripping with magic items, me and my friends started at 1st level. In such a case you might see the lower level folks get enough xp to jump levels...if they survived.

Look at the last Rise of the Runelords adventure and it's really hard to have a breaking point. So we did have enough to double for one character. You could award once reached and just level during play, but those that like to wait for a break in the story may find this happens.
Waiting till the end of a session is different than waiting for the end of a story arc.
Now that could bring into question things like "How does a spellcaster with a book get new spells in the book if they are out in the middle of nowhere?", but really, who cares? Lots of details of leveling up need to be hand waved already - like at what point did the fighter practice that new sword move? Or when did the rogue practice evading fireballs? Or the linguist study a new language? All of those things are presumed to have been occuring while they were gaining experience. So maybe the mage already had the spells written down, but just didn't understand them yet.

I'm now picturing the massive battle against the BBEG and his minions coming to a halt as killing one of those minions gives the party a level up and they want the boost before resuming the fight against the boss. "Time out, time out I just gained a level and want my 4th level spells to fight you with."

Reminds me of Asterix, when the Romans invaded England - and the English paused battle at 5 for tea time...
Gaining two levels with one encounter, hmm. To instantly get from level 1 to level 3 you need a bit more than 3k XP, even if we assume you are very close to level 2 already. Since XP is divided by 4 usually, you need more than 12k XP. And that's a CR 11 encounter. Maybe if you catch them asleep and happen to be a strong scythe wielder...
