If one has multiple intentions, do I pick one or do they appear as all? Would seem awfully suspicious if there was more than one.

Usually, if the investigating party fails a check, they only detect/identify the cursed item as the item it appears to be. If the item it appears to be would have multiple readings, then the cursed item probably will as well.

If they succeed at determining its true nature, then they should know everything it does, ie. belt of weakness and arrow attraction and teleport 1/day (with 50% chance of mishap).
