If I was a Paladin, and I had the Aegis of recovery Item, Determination on my armor, and Hero's Defiance prepped, would all three of them activate as soon as I hit 0 hp for a "Super revive"?
If I died how would the other 2 heals work in combination with the Breath of life?
Edit: Can I choose to not activate any of these and save them for another revival?
Edit 2: If I were to get determination on a suit of armor and a shield, would they both work together?

Hero's Defiance appears to happen before you are reduced to zero hit points. If that was sufficient to keep you above zero, then the other two effects wouldn't fire off.
Both the aegis and determination fire off automatically when you hit zero, so they appear to both go off at the same time. I believe the general rule is if a two things would happen at once, the owner of the effects gets to determine the order (although I don't have a specific citation for that atm.) Therefore if breath of life would keep you alive, the aegis would be able to heal you more afterwards.
The only question really is if the aegis would heal you if the damage would be sufficient to make you dead. If you just had the aegis, and took enough damage to kill you, I don't see anything RAW that would allow it to heal you, as healing doesn't work on a dead person. However it possibly could be interpreted as being the equivalent of having a readied action, in which case the healing would apply before you gained the dead condition.
I would personally allow the Aegis to heal you during the period between dying and dead, preventing death if it was enough, but I'll freely admit the RAW support for that is very thin.

I mostly agree with Dave, though I'd be inclined to err in favour of the player's best position.
I wouldn't be as stringent with the wording, such as Hero's Defiance saying "the instant before" and the other two not using that wording, because I don't think that's in the spirit of each of their effects.
The way I'd do it: if a player dropped below 0, they could choose which effect goes off first. If it's still not enough, they could choose the next one. If it's still not enough, they could again choose for the next one.
If you've got the dead condition, you can't be healed anyway, and they don't go off (and the aegis doesn't crumble to dust).
