My son and I succeed at Brigandoom and at the bottom of the scenario card is text "Reward: Each character draws a random item from the box".
What is the point of drawing an extra item? Aren't we supposed to rebuild our character decks to match the numbers on the character card? Wouldn't we just have to return this item to the box (or at best trade it for an item that we got in our initial deck)
All three of the starter set scenario rewards look fairly useless.
I assume I am missing something.

If I understand your question correctly, your second guess is right, you will need to get rid of either the item you draw, or one from your deck (unless you banished a card at some point, then you won't have too many). Say you had a Thieve's Tools in your deck, and your random item is a Masterwork Tools. That's definitely an upgrade, and you now get to bring that Masterwork along in your deck instead of Thieve's for the rest of the adventure path until you decide you don't want it. It's just like acquiring a card during gameplay, only you don't make the checks you just get the card.
Are you thinking that after every scenario, you rebuild your starter deck? If so, you only need to be making sure you have the right number of cards, what those cards are don't matter. Of course, you can only take cards with the Basic trait to build your deck until Adventure 3, at which point you can take anything 2 adventures before you or older. Always build what you can with the cards you have, and only go to the box for more if you don't have enough cards available.
Maybe you pick up a Deathbane Light Crossbow as your random Weapon, now you can get rid of that Dagger or Light Crossbow you've been dragging around, etc.

Yes, you'd have to trade it for an item you have in your deck, if you want to keep it. Chances are it won't be a useful item, but you might get lucky and draw a good item, like the Masterwork Tools. Since your initial deck was all Basic cards, there is a chance that you will draw a better item as a reward.
Generally, the "draw a type of card" rewards are seen as less good than pieces of loot or feat rewards. That said, I have on occasion drawn a good reward from them. Note that when you finish the third scenario, you also get the Adventure Reward (a Skill feat, if memory serves me correctly).

Not exactly.
The cards in the Perils of the Lost Coast adventure are "Base" cards (with a "B" in the upper right corner, or a "C" if you are also using the Character Add-On deck.
This is different from cards with the "Basic" trait. These cards say basic in the traits list on the upper left. You can only build your starting decks with cards that have the Basic trait. Base set cards that do not have the Basic trait may not be used in your starter deck, and can only be acquired through adventuring.
If you are using base set cards other than basic to build your starting deck, then I could see where you might not be too excited with rewards from the box.

So it looks like the rewards in the APs actually are worth something but the rewards in the starter scenarios make no difference since they are just the basic cards.
The character decks when you first start playing RotR (i.e when you start Brigandoom!) are limited to Basic trait cards, but the rewards and locations are not. That means the random item you might receive could be the awesome Crown of Charisma. The Crown of Charisma lacks the Basic trait, so it can't be started with. But it is a random item in the box during Perils of the Lost Coast, so it might be something you pull for the reward. (Don't confuse the Basic trait in the upper left corner of the card with the Adventure Deck number B, for Base Set, in the upper right corner of the card.)
There is a chance the reward could be awesome, and there is a chance the reward could be useless. It is random. And even if it isn't awesome for your character, it might be good for another character that finished the scenario with you. So you could give it to them.
I once pulled Poog of Zarongel as a random ally for Local Heroes. It was pretty awesome.

You should not be including any cards with a higher adventure number than what you currently play. If you are playing a B Adventure and have all decks in the box, and you draw anything 1 or higher as your reward, return it the box and redraw until you have a B. No cards over the current adventure should ever be used.
