But note that most bags of holding are 2'x4'. So imagine a pillowcase that when you flatten it out is 2'x4'. I doubt you'd be able to get many adventurer types into the opening, although perhaps Small-sized creatures might be able to make it. (Btw, don't forget to count familiars as creatures!)
I had a party do this, but only one person was outside the bag (it's a long story for another day). When that person was knocked unconscious, the party found themselves trapped in the BoH, waiting to suffocate!!
I had to pull some GM fiat to get them out of that one or it would've been TPK right there. Nothing glorious about it either, just a little bad planning. :)

Why didn't they simply leave the neck of the BoH open so they could breathe? Otherwise, what was stopping them from simply opening the bag from the inside?

A bag of holding entrance should have a 4' circumference (2' top when flattened x2).
As it is a clot sac you can easily shape the entrance as it better fit you, so a 3' long for 1' and 4" large opening or something similar.
it would be at least a full round action entering it for a medium creature but not impossible as it become larger immediately after you cross the entrance.
Heavy armor will change that.
The easy way to test that is to use a 4' long string (after knotting it) and trying to pass it from your head to your shoulders. Taht is how flexible the entrance is.
In the older versions (2ed and before) rupturing the bag/portable hole. would have sent you in the Astral or Ethereal plane, now you are lost forever. A plane shift from inside the bag should still work.
Beside that I am not convinced that it can't be opened from the inside unless the opening was closed with a knot. You know where the opening is, you have the side to push against so it should be possible to open it. It the bag was rolled up/folded and put in a pocket with the opening in the inside of the pocket it will become difficult, maybe requiring some escape artist roll or similar, but still not an impossible task.
Fox leaving the bag rapidly after a teleport the carrier could simply turn it inside out.
Teleport: "You may also bring one additional willing Medium or smaller creature (carrying gear or objects up to its maximum load) or its equivalent per three caster levels. A Large creature counts as two Medium creatures, a Huge creature counts as four Medium creatures, and so forth."
By that description your familiar will count as a medium or smaller creature even if it is tiny.

In regards to the teleport spell, would creatures carried in a portable hole or bag of holding count against the limit of "passengers" a caster can bring along?
We use this method of travel in one of our campaigns pretty regularly, but we team it with reduce person which reduces the weight of the target by a factor of 8 (IIRC).
